About Avir at Meadow Creek

Avir at Meadow Creek is an 80-bed skilled nursing and long-term care facility in San Angelo, Texas. It serves patients recovering from surgery and those requiring extended rehabilitative care.

Family experiences at Meadow Creek vary significantly. Some families praise the nursing and therapy staff, particularly noting compassionate care and focused rehabilitation support after surgery. The kitchen and food quality receive consistent compliments, with one family commenting that their father "always bragged on the food." However, other families report serious concerns: inadequate cleanliness, staffing lapses in patient monitoring and hygiene routines, and critical documentation issues such as lost Do Not Resuscitate orders. Concerns about adherence to physician orders and responsiveness to family feedback appear in multiple accounts.

The facility holds a 3.6 out of 5 Google rating based on 44 reviews. The disparity between five-star and one-star reviews suggests inconsistent care experiences, with post-surgical recovery outcomes and daily monitoring practices being particular points of concern for some families.

This place was a nightmare!! I WOULD NEVER EVER RECOMMEND THIS PPACE. They left my LO alone in the shower, he was a fall risk. Never went to turn him. He had back surgery. I would get there at 2 or 3 pm and he would still be in bed!!! They do NOT to be called out for their mistakes!!

I am incredibly grateful for the exceptional care my father has received at Meadow Creek Nursing Rehabilitation. Admitting him to a facility was a daunting decision for our family, but the compassionate support from the nurses and CNAs has made all the difference. Despite some changes in leadership, there remains a consistent commitment to quality care that shines through. Nurse Ramona Essary and Georgianna Escamilla, the Director of Nursing, have been instrumental in making this transition smoother for us. Their open communication has alleviated our concerns, especially given our distance from the facility. While some may share negative experiences, we can wholeheartedly say that our journey here has been overwhelmingly positive. The dedication of the entire staff deserves recognition, and we extend our heartfelt thanks to them and the administration team for their unwavering support. They truly go above and beyond in providing care that our father deserves! With gratitude, Dr. Willie Black, Jr. and family

WARNING‼️‼️‼️DO NOT USE THIS FACILITY ESPECIALLY if your LOVED ONES ARE THERE TO RECOVER FROM SURGERY! Dad recently underwent brain surgery and Shannon Medical recommended he be in a skilled nursing facility to finish the remainder of his treatment and therapy. Out of 7 nursing facilities we decided to with Meadow Creek as this was the best option for my dad as his medical insurance would cover everything while being there. My dad has been there for about 2 weeks and has gotten worse!!!! This place was supposed to take GOOD CARE OF MY DAD AND THEY FAILED TO DO SO!! My dad should have been getting better especially if he has a central line to administer antibiotics. Well now, I’m starting to think that the NIGHT MEDICAL STAFF is the one not taking care of my dad as my dad stated that a woman by the name of Norma was making fun of the way he is mumbling. She mocked him! Which is not very ethical of someone with "medical background". She also got upset with dad for wearing a sweater. Is my dad not allowed to wear a sweater!!! The fact that she is doing this to him and possibly other residents is beyond humiliating. My dad is fully aware and is not there by choice. Like I said, he’s there to recover from brain surgery not to live there. If NORMA is capable of mistreating a recovering patient, I could only imagine how she treats the residents from this facility.

Terrible place to have to take your loved one. It's gross. It never actually gets clean. The staff (at the time) were lazy and ill equipped to care for their residents, at times going against actual doctors orders for care and treatments. LOSING DNR orders, which is really a big deal if you dont want the paramedics to have to do CPR on your loved one an hour after theyve already passed! That call was traumatizing. If we had other options we would have kept my grandma out of there.

My dad went here for rehab after back surgery and we were very well pleased. The staff was very friendly and focused on dad’s recovery, most especially the therapy team. Kudos as well to Willie, the chef, as dad always bragged on the food. Special thanks to Bekha for providing a tour and getting us ready for dad’s admission. If we ever should need services in the future Meadowcreek will be our first call. Thank you to the entire team for the exceptional care!!
