Do NOT allow your loved one to be admitted to this facility. I am a retired Rehab nurse, so I have experience with the elderly and how rehab facilities are supposed to care for their patients and plan for discharge. When my sister was admitted there, I called immediately to inform them she would not be able to return to her apartment, and would need assisted living after discharge. I also told them (nursing staff) that she is in cognitive decline. I requested to speak with the nurse practitioner on several occasions, with no response. After my sister’s insurance ran out, they finally moved on discharge plans to assisted living. We (family) were given days notice to arrange movers for her furniture and belongings and start the process of clearing out her apartment. To add insult to injury, because they dragged their feet and didn’t start discharge plans earlier, her insurance has run out. Avina now expects her to pay for her last several days there out of pocket.

Went there to visit a friend. All seemed happy. Place was very clean, and smelled good.
