I have lived at Aviata @Orange Park this time for a year. However I was here for 3 years, left for a little bit and would only come back here when I was told I had to get therapy as I have MS. I had such amazing therapists. The Nursing staff and CNAs are nothing but professionals. The grounds are impeccable, and kept up so nicely. The maintenance staff is always on the ball keeping everything in working order and updated. Everyone in the office levels from the Social Worker to the DON and Administrator do their jobs with the utmost respect to the facility rules and the responsibility to the people involved. The entire team at Aviata at Orange Park are in my opinion the greatest!!!

While the majority of the staff was AMAZING, there are some that do not do there job to the fullest. However, any grievances were handled quickly. Jennika, Jazz, Destiny, Lisa, the activities staff, admin staff, social services staff and of course the OT & PT STAFF WERE CARING & INCREDIBLE! The kitchen definitely needs work, orders were wrong often, special dietary needs not always followed and many times the menu did not match what was served. The facility itself needs A LOT of work, old equipment, bugs, and unsafe flooring in the shower are a concern. Overall, my experience wasn’t horrible but there are things that need work.

If you are looking for a senior living center that prioritizes both aesthetics and atmosphere, this is it. 2 main things stood out to me. First, the environment. The facility is very clean, and you can tell they’ve invested into the look of the place. The entrance and new furniture give it a welcoming, upscale feel along with the new landscaping. Secondly, the culture and employees. Beyond the looks, there is a real 'people-first' energy here. The residents are active and smiling. Most importantly, I’ve noticed the team here is incredibly open to feedback. They don’t just listen, they apply changes to ensure the facility keeps improving. It’s that growth mindset that makes this a great place to live.

I had a great experience with Aviata at Orange Park. The staff is attentive, compassionate, and truly dedicated to providing quality care. The team communicates effectively with families. I would definitely recommend this facility to anyone in need of skilled nursing or rehabilitation services

Someone who worked at this facility took my phone and left a false review of 5 stars. If I could give this place 0 stars I would this is the worst place I have ever been in my life. I was here for almost a year and two of my friends died from how unclean it is due to bacterial pneumonia going around. They refused me to go to the hospital I had to beg because I could not breathe I finally got to go and had to be in ICU. The aids are all rude and mean and half are too busy flirting with the male nurse to care for patients. Some people had fell and no one came for so long I had to wheel myself up to get help for them on the floor. No one comes when you turn your light on and most patients are very neglected but have no family to keep the place accountable. It is absolutely horrible there. My clothes and shoes got stolen, Tiffany was supposed to replace my shorts and did not so I have no shorts to wear after leaving there. The place I am in now is wonderful but please if you care for your family do not send them here. It's nasty and unclean, use the same mop to clean every room with c diff outbreaks and who knows what else. I think they are committing fraud with people's money because a lot of people do not get their monthly allowance. They don't come check on you when they are supposed to or give you baths and they act like they should not be doing their job. Horrible horrible horrible. This place needs to close it has gone way down hill.
