For the health of our patients, visitors, health care providers and communities, we ask that people do not visit our facilities, including senior services and long-term care locations. For mother/baby and pediatric patients, visitors are limited to a designated partner or primary caretaker/guardian (only one at a time). DaySTAE activities

Activities in the program are carefully chosen to hold participants attention and promote positive feelings. We use art, music and storytelling to engage participants, helping them maintain cognitive and other functions.

•    TimeSlips is an activity that replaces the need to remember stories with encouragement to imagine them. Our facilitator asks open-ended questions to guide storytelling, and responses are woven together into a story. Participants enjoy the stories and the feeling of success gained in contributing to them.
•    Memories in the Making allows participants in early to middle stages of dementia to recover and communicate memories through painting and drawing. They enjoy the creative process and the ability to express feelings associated with their memories.
•    Drumming circles are an empowering and spiritual experience that allows participants to feel and share their potential through rhythm. They share and explore feelings and connect with their most basic selves, regardless of their level of functional ability.

I only gave 1 star because you couldn't give it a 0 star or negative rating!!! Under NO CIRCUMSTANCES do you want to put ANY ONE in this facility that you love. My husband arrived on 6/03 from a rehab center and I had him taken out of there after calling 911 on 6/06 because he declined immediately after arriving didn't eat, or try to talk or move. I would almost get physically ill when I would step in to this place. The smell was UNGODLY and there were patients roaming the whole facility inside and outside. Had I not called 911 that evening there is no doubt in my mind that he would no longer be alive. The EMT's immediately tested his blood sugar and it was 55 he is not a diabetic but hadn't eaten or drank in almost 3 days so that did not surprise me. The immediately administered an IV and gave him Glucose water to where he did become somewhat alert but could not tell them his name, birthday or where he was at. One of the firemen told me he hated coming to this place and they had been trying to get it shut down well hearing that made me even more convinced that I was getting him out of there if I had to take him out myself!!! They rushed him to Good Sam where he was from 6/06 until 6/22 and the drs told me he had, had another stroke within the past week!!! I would put my life on it that he had it while he was at this HELL HOLE had I not took it upon myself to call 911 he would be dead or in a coma!!! DO NOT I REPEAT DO NOT PUT YOUR LOVED ONE IN THIS PLACE or for that face your worst enemy!!!

My mom spent 30 days here for rehab. When she got there, the room was not clean and the water in the shower kept running. When she left the room was not clean and the water in the shower was still running. My mom was very unhappy with the service that she was provided. So I removed my mother from this place and I do not recommend this place for anyone.

I would not recommend Aventura at West Park to anyone. My loved one was discharged without any prior notice or communication from the facility. Because of this lack of coordination, they were sent home without any home care services in place and missing essential medical equipment. The experience during the stay was equally disappointing. The staff was consistently rude and "nasty" to both the patient and the family. Additionally, the food quality was poor and often unpalatable. This facility failed us on both a clinical and professional level. Avoid this place if you want your family member to be treated with respect and safety.
