Our Mom needed rehab in May 2024 after a fall and surgery at CMC, and Aventura Creekside had an opening. I had a concern and quickly received calls from Karen K, Cole and April, who immediately took action and great care of our mom. Each are true professionals who were reachable by phone if needed, and who listened and exhibited sincerity and caring. Mom said that Jason in physical therapy was professional, attentive and caring and helped her recover, and I found his caring manner and excellent work made a difference to our Mom.

True Gem in terms of care and attention, to not only their residents but the overall atmosphere and facilities. The activities and programs put on by the staff truly make it feel like a second home. Couldn’t ask for a better experience for your loved ones.

This facility is very quaint and cozy. The staff are fantastic and treat all residents with respect, always keeping their dignity in mind. The Administrator and his team are so kind and compassionate! They handle any concerns you may have in a timely manner. I couldn’t ask for a better place to care for my loved one. If you’re in the Carbondale area and in need of skilled nursing care, this is your place!!

The people are rude to their patients & leave them in their own piss also they don't change the bedding if you couldn't hold it any longer. You would be laying in your own piss etc for between 2-4 hours or so. You would be lucky to be able to eat something. This place is a joke. I may not experience the place but my mom is at the place as a patient. What a disgrace oh if anybody wondering if you don't like the place & plan to sign yourself out expected to pay $250 (a day) as of how many days you stayed if the insurance didn't pick it up.
