My mother received excellent care at Autumn View after a hip fracture. The aides were especially attentive and kind. The facility is clean and the staff is friendly. She was there for 2 weeks and was given PT and OT daily which made a huge difference in her overall recovery. I would recommend this facility for anyone recovering from a surgery and in need of rehab.

Wish I could give 0 stars. I am having my mother transferred out of this nasty facility. They neglect her beyond belief. Hr and a half to 2 hrs wait time to use the bathroom. They complain of putting her socks on her feet. She hasn't even been there a week. They refused to transfer her to another facility until we got the other facility involved with getting paperwork. Its bad when even the supervisors and head nurse knows that on nights and weekends where there is no supervisors on that nothing gets done and residents are ignored. I will NEVER send anyone there not even for recommendation.

Ants under beds because of food falling from residents. Clothing not changed up to 5 days. Water pitchers empty or not in reach. No cups for drinking. Bedding not changed for hours. Lunch room, bathroom and section near administrative offices clean, bright no urine smells. Other areas filthy, smell of urine, public bathroom loose faucet fixtures, crumbling floor tiles.

My elderly parent received and continues to receive physical therapy here due to falls , hip fractures and functional declines. I couldn’t be more grateful for the care Olivia and Leah are providing ! These ladies are incredibly patient, kind, and respectful, always taking the time to explain the treatment provided and ensuring my parent and family are comfortable with the plan. They treat my parent with dignity and genuine compassion, which meant a lot to our family. We have saw real improvements in strength, balance, and confidence. I highly recommend this clinic for anyone seeking physical therapy for an older loved one. The patients and Autumnview are lucky to have the both of you!!!

My husband was recently admitted to Autumn View for rehab following major abdominal surgery. He arrived unable to stand or walk and had a long history of poor eating, so we knew his care would be challenging. Overall, our experience was mostly positive, with several staff members going above and beyond for both him and our family. Most of the nurses we encountered were genuinely caring and professional. Samantha, a day nurse, was outstanding from the very first day. She consistently went above and beyond at every step, making sure my husband was comfortable, informed, and treated with compassion. Donna, the evening nurse, was also excellent—very attentive and always diligent about keeping me updated. In my husband’s last two days, Kelly, a day nurse who provided end-of-life care, showed remarkable knowledge, sensitivity, and respect. She treated him with true dignity, and I will always appreciate that. The end-of-life care team overall was very kind. They checked in often and made sure the family had anything we needed during a very difficult time. The physical therapist, Josh, deserves special mention as well. He consistently brought a positive attitude, encouraging my husband to move, eat, and stay engaged. He was both kind and motivating, and clearly had a good heart. In terms of the facility itself, most staff members were respectful, kind, and knowledgeable. That said, I did notice that staffing seemed low at times, especially on weekends. The building is a bit dated and the rehab section tends to run warm. Parking can be limited, and because the facility sits on a busy road, getting in and out of the lot can be challenging. My biggest disappointment was the food—unfortunately, it was quite poor. Much of it was mushy, and several other patients even commented to us about the food quality. On a positive note, the building does offer several nice areas for family visits, which we appreciated. Overall, while there are areas that could be improved—particularly the food quality and staffing levels—there were also many compassionate, dedicated people who made a difficult time a little easier. I’m grateful to the staff members who truly cared for my husband with respect and kindness.
