This has been a great experience here at Northwest. Sambria and Jose in therapy are great. The Director of Nurses is so caring and compassionate. She is the reason I stayed and completed my care. This is a great place.

When we first came to the facility for my aunt the nurses were attentive and welcoming. Wound care explained what was going with my aunt, the disease process, and how we can help with the healing process. I truly appreciate them helping us out.

Do not leave your parent there! I’m filling a report with Medicare and the state. The director is very quick to return calls and assist you as best as possible and some staff is nice but there is too much neglect. Makes you wonder what goes on when you are not there. Remodeling/construction is visible but a bandaid at best, the facility smells like urine, vents on the AC have some kind of mold, there is very limited staff at night, and the poor patients are complaining left and right about the way they are being left untreated. My Father went in a Wednesday late night left in a wet for over an hour I had to call, he had a fall Thursday hit his head- due to him trying to be more self sufficient since they never came to help him they took him to the ER by Friday we were pulling him out and arranging his transfer, finally Saturday morning we found out that he he was left wet all night completely uncomfortable then they sat him in the most uncomfortable chair for 3 hours telling him they will be there in a minute thank God we got him out!

We had a very disappointing experience with the admissions process for our elderly mother. We began communicating months in advance regarding placement in a Medicaid LTC bed. As our mother’s needs became urgent, Olivia (head of admissions) outlined the required financial and clinical documents and provided her direct contact information. We submitted everything requested just before Christmas, after which communication completely stopped. Despite multiple emails and texts from our family and follow-ups from the SNF social worker, we received no response. While we initially assumed the delay was due to the holidays, it is now January 13th and still no follow up. At no point were we told a bed was unavailable; we were led to believe placement was likely once the process was finalized. This lack of communication resulted in unexpected SNF costs and left us scrambling for alternative placement under urgent circumstances. When it mattered most, reaching admissions was impossible. A timely update would have been far more professional.

We had a very disappointing experience with Olivia head of admissions and our elderly mother. I’m only leaving a five star so it shows up in the recent reviews. We started communicating with Olivia months ago about the admission process for a Medicaid bed in LTC as it got closer to our mother needing a facility she gave us her cell phone number and told us what the steps would be; what financials and what clinicals would be needed. We got all the required documents in order to her right before Christmas and then we just stopped hearing from her. She stopped responding to emails and texts from us as well as the social worker from our mom’s SNF. We thought perhaps it was the holidays but we’ve still yet to hear back from her and it’s January 9th. There was no indication of there not being a bed available, in fact quite the opposite. We were told that as long as she met the requirements (which she does according to her social worker) that once the interview process was finalized she could be placed. This is very upsetting because had we known that legend oaks didn’t want her or couldn’t take her - we would have had other LTC facilities picked out. We got strapped with a huge daily bill from SNF because we were thinking any day Olivia would respond. Now we’re faced with taking our mother home to live alone and waiting on another LTC to hopefully admit her. Hopefully our mother doesn’t re injure herself and end up back in a hospital. It would have been more professional to at least communicate their unwillingness to take her in a timely manner instead of stringing her and us along. Getting a hold of Olivia proved to be impossible when it counted. I hope others have a better experience when dealing with them.
