This place is a nightmare. The staff is lazy and don’t care for the residents the way they should . They ignore the patients at night when they call them , they don’t keep them clean. I’m so frustrated, I feel like I’ve failed my mother by letting her stay for rehabilitation . And to top it off they don’t even clean the rooms daily.

My mother needed extra care after two surgeries that I was not able to provide. Zachary Manor provided the skilled nursing that she required. I will admit that I was very skeptical about sending her anywhere but home. I visited several facilities and after consulting with the care teams at the hospital, we chose Zachary Manor. My mom was there for 19 days and was treated with the respect and dignity that she deserved. The entire staff was so kind and loving. I was able to visit often and was always impressed with the relationship that the staff built with the patients that were staying there. Sending your loved one to any facility for care is always scary and often times heartbreaking. Knowing that she was getting the care and attention she needed was a relief for me. I can honestly say that it was the right place for my mom for the care that she needed. She is home now, but she does plan to go back every once in a while to visit the friends that she made that are living there and to also see the wonderful aides, nurses and therapists that got her back on the road to a full recovery. For those at Zachary Manor that read this...Thank you from the bottom of my heart for your care and kindness.

I'm hurting. My mom wasn't here not even 2 whole weeks before she passed at this facility.When I first walked in the place I wanted to throw up. It looked like something off a scary movie. Before my mom got there she was in good health, had a good spirit. Was only there for some rehabilitation due to her breathing and walking for a long period of times. My mom was saying that she was taking alot of pills which she really didn't know what some of them was. Well unfortunately she died within a week and some days. We heard two different stories. The food was horrible. The muffin was so hard, like how do you expect someone to eat that. I wouldn't bring a dog back to that horrible place. Whatever yall did to my mom only God knows but yall will suffer. All of yall.

The worst possible option for nursing home and rehabilitation in Zachary, LA. My mother was there for cardiac rehab. No running water in the room - she was brought a bowl of luke warm water and a rag when she was offered an opportunity to bathe. They rarely changed the sheets. There were “residents” roaming in and out of her room at all hours. She refers to it as the “funeral home” because just outside her window in a shed on the property was an actual CASKET. In plain sight. Apparently one of the managers loved to really go all out with Halloween decorations. A bit tone deaf for a nursing home /rehab facility. We were told her stay would be 100% covered by Medicare. Received a bill from a collections agency nearly 18 months later. Zachary Manor never billed her for anything. STAY AWAY.
