Some of my experience and my family experience hasn't been the best! When it comes to the well being of my brother ouf love one! We would like to see more staffing who whole heartly care for their residents then just to be there for a check ...it shows and some people whom are in these facilities didnt ask to be there and only wana be treated be treated with care and as such..

On June 28, 2025, a motorcycle accident changed my life, leaving me a double below-the-knee amputee. During this challenging time, Willow Valley treated me with the utmost kindness and courtesy. I formed great relationships with the staff and most of the patients here. The team truly does their best to accommodate and care for every individual. I am especially grateful to the staff coordinator, main director, and social worker for being so dynamic and supportive. I am writing to formally report and escalate multiple serious issues regarding the care and conditions my mother experienced immediately upon her arrival at your facility on March 16, 2026. What has occurred within her first 24 hours is unacceptable, unsafe, and deeply concerning. My mother arrived around 4:00 PM. By 5:54 PM, she was already calling me in distress, wanting to leave due to the conditions and lack of support. Neglect of Basic Care and Safety My father requested a pull‑up and a bedside commode riser so she could safely use the restroom. While he was there, another resident repeatedly asked my family for help because staff were not responding. When my father addressed this, staff ignored the resident. This alone raised immediate concerns about patient safety and supervision. No Heat in Her Room Overnight My mother’s heat stopped working during the night. Staff did not check on her until approximately 3:00 AM, at which point she informed them she was freezing. She asked for the door to be opened so heat from the hallway could warm the room. She was not moved to a room with working heat, nor was she provided extra blankets. At her age, prolonged exposure to cold temperatures poses a real risk of hypothermia. Leaving her in a cold room all night was negligent and dangerous. Room Not Properly Assigned As of 3:30 PM today, March 17, 2026, the room tag still displayed another person’s name. This is unacceptable and reflects a lack of attention to detail and patient dignity. This also raises concerns about medication safety, as staff could have mistakenly administered another patient’s medication to my mother. Lack of Communication From Staff During the care plan meeting at 11:30 AM today, my father informed the social worker, Tesha, that he was extremely unhappy with the facility’s performance. We were told she would follow up. By 3:00 PM, we still had not heard from Tesha. I had to go to her office myself to confirm whether she had contacted the facility we are transferring my mother to. Additional Failures in Basic Care My mother did not have a water pitcher in her room. I had to request water myself. She did not receive a bath upon arrival, nor was she given one today. She did not receive a TB test upon admission. She pulled the call bell in the bathroom this morning and no one responded. She sat on the toilet for an extended period until housekeeping—not nursing staff—found her and had to get a CNA to assist her. . Her window blind does not work, preventing natural sunlight from warming the room, especially important since the heater was not functioning. The food quality is unacceptable. She could not identify what she was served, and it was overcooked and dry. We had to provide her meals today and will continue doing so. The toilet seat riser requested yesterday was still not installed by 2:30 PM today. I had to request it again at the nurses’ station, and it arrived 20 minutes later. Positive Note The wound care doctor was exceptional—professional, attentive, and compassionate. She deserves recognition for her care. However, the overall conditions and lack of responsiveness at Willow Valley Center are alarming. My mother has not even been in your facility for 24 hours, and the number of failures is staggering. This level of neglect warrants a full investigation by the NC State Board. We are in the process of transferring my mother to another facility immediately. I expect acknowledgment of this complaint and confirmation that these issues will be addressed. Do not put your loved ones here, even if it’s just for rehabilitation. Nurses and rehab do not communicate. My dad requested yesterday that the nurses relay information to PT/OT. This situation is unacceptable and must be taken seriously!

The place might be a little dated, but the care you receive is excellent. The people you deal with most are CNAs or CMAs. They are professional and friendly and did more for me than I would have expected. The nurses are very attentive and will do what they can to make your stay better. The physical and occupational therapists were above reproach. I went in unable to stand and left there able to walk on a cane. After a week or two in, I felt more like they were more my friends than my therapists. I progressed a lot and cannot say enough good about them. The facility seemed to be organized and well run. I’m sure there are some that might be really organized and run by the books, but the Willows was fine by me. All of the staff were friendly and helpful and I’ll take that over strict organization. One little flaw. When I was in, the food was absolutely horrible. I lost quite a bit of weight because I couldn’t eat that dog dookie. The last couple of weeks I was there, they hired a new food service director and she made some changes that helped. I think by now the offerings have improved. If I should have to go into rehab in the future I would want to go back there.

7/25 Second time here provided a better experience. The employees I encountered were more pleasant. The resident I saw said that the case manager actually apologized to her for an incident and commended her for advocating for herself. That speaks volumes to me. Not many employees will apologize and take accountability. I got to see the dining room this visit. It was nice and bright, clean and a great space to sit and chat. First visit: called one day to check to make sure one of the residents was in fact there. The lady answering the phone seemed annoyed and transferred me up to the second floor. When no one answered there, it rerouted me to pissy lady. She begrudgingly took my information and said she’d give it to the resident. Guess what? She never got it. The second time I called (different day) a lady named Amanda answered and was very pleasant and helpful. When I arrived (having been my first time ever there), I was surprised with how nice the lobby looked compared to the outside of the building. I noticed a score of 88 posted in the lobby but failed to get a photo. As nice as the restroom, lobby and all was, once you got to the floor where the patient rooms were, it was a shock how dirty the carpet was and just the overall feel of it. The employees all looked miserable with the exception of one that I encountered in the day room. The resident reported that this place is awful. That two times the kitchen never even brought her food. She said that the evening shift employees usually sit together in the day room while patients are yelling for help. 😢 As I was saying goodbye to the resident, one of the staff members gave me an ugly side eye. They just all seem miserable there! I would not recommend this place at all. They can pretty up the decor all they want but I’m sure the loved ones of those residents would rather it be an ugly building and receive great, compassionate care!!!
