My mom has been in the facility a couple times. Always a good experience. You can rely on the staff and the place is always clean. After coming from a place like Baptist in Scotia, Washington Center deserves all the credit.

Everyone I have come in contact with has been great. They take the time to address all of my question. They have been great with my husband who can be very difficult to deal with. Thank you very much Sandy Harvey

If you’d like your loved one to lay in their own sewage for 3-4 hours at a time while call bells go unanswered, this is your dream facility. Meals I wouldn’t offer my dog. Pasta with a side of pasta with a glass of water and a side of cold coffee, one of the better offerings. There are a few good people that work here, by which I mean actual nurses, but unfortunately in the greater structure horribly overwhelmed and very likely underpaid. Don’t let the 5 star Medicare ranking fool you. Horrendous.

If I could give this facility 0 stars I would. I have been in health care my entire career; from nursing homes, to assisted living, EMS, and hospital settings and I have never once encountered the horrors that is the Washington centers. My grandfather was there for rehab, when I walked in I found him covered in feces, all over his bed. His nasal cannula on the floor. (Mind you he was there due to an infection, so let’s take a dirty cannula off the floor and put it into a mucosal membrane then wonder why we have an infection….) Crumbs all over the floor. No call bell in site. His beverage not within reach if he wanted to try and get it. Not dressed, not covered, laying helplessly. When I asked an aide if she was going to give morning care to my grandfather she stated “he’s not my resident I’m not touching him.” While he lay there soiled. I went and spoke with the administrator, who said it was unacceptable and she would have someone there promptly to care for him…. I waited another hour and nobody showed to care for him. I’ve heard horror stories, much like many facilities around the area… but this was atrocious to walk in to. I wouldn’t place a family member in this facility. Also after research found that their last convicted case of neglect was 9/12/2025. Just a few short months ago where they were fined 14,500 because they were found guilty of neglect. It clearly shows!

I do not recommend the rehabilitation side to anyone not even my dog....the care for there patients is absolutely horrible...My mother was there for rehab and they did a poor job of rehabilitation .she fell 2x while there as they had her sitting in a Wheelchair ,and claim they had no other chairs....The care is just horrible.The discharge doctor was just as bad....I believe the staff should be well trained in looking for things especially Sepsis!!!!!!! But bussing them feom wherever and its a body they dont care....I feel so bad for patients whom have no advocate to speak for them....
