We strive to make every moment count for our residents and patients at Villages of Jackson Creek because we believe in celebrating life at all stages.

Short Term or Home. Our residents and patients come to us for a variety of reasons.  Many are short-term, having left a hospital needing skilled nursing and/or rehabilitative care to get them back home.  Other residents have chosen us as their home for assisted living care or long term nursing care.

The Villages of Jackson Creek’s private and spacious campus is home to a 120-bed Health Care Center and a 52-apartment Assisted Living Center. Our campus has a restaurant-style dining room as well as recreational amenities like a theater, ice cream parlor and a salon.

We are dedicated to offering our residents and patients the independence they want with the support they need.  In fact, all Villages of Jackson Creek partners prepare for their workday by reciting one of our 20 promises each morning.  We want to provide the best rehabilitative and clinical services based on the activity and care needs of each of our residents and patients.

The Villages of Jackson Creek’s private and spacious campus is home to a 120-bed post-acute 24-hour skilled nursing Health Care Center. We see many individuals who need skilled nursing care after a stroke, joint replacement surgery, a cardiac procedure or a serious illness.

The Villages of Jackson Creek Memory Care is an extraordinary place where compassionate, caring and well trained professionals provide a variety of care to our residents living with various forms of dementia and Alzheimer’s.

The Memory Care Program at the Villages of Jackson Creek is designed to enhance the quality of life and personal dignity of residents living with Alzheimer’s and other types of dementia. Our accommodations are secure, beautiful and specifically designed to ensure our residents are comfortable and able to enjoy life. Since the Villages of Jackson Creek Memory Care is part of a health care community which includes an Assisted Living center with skilled nursing services, our residents receive 24-hour nursing care from our licensed nursing staff.

The Memory Care Village consists of 70 spacious apartments where we provide the secure, safe environment that ensures for each resident, optimal health, emotional and spiritual well-being and provides true peace of mind to family members.

Our secure memory care center is supervised 24 hours a day, seven days a week, with skilled care professionals monitoring our residents and giving their family members peace of mind. Our team members promote an active and high quality of life for our residents, using the most successful activities and therapies to improve each resident’s daily quality of life.

Through the Village’s innovative “Memory Lane” corporate branded social model, we offer person-centered dementia care.
