The staff are all amazing. They answered all our questions. Walked us through the process and made every effort to make our loved one feel safe, welcome and cared for. Anytime we had questions they were amazing and got us all the information we needed. The environment created by the staff truly makes residents feel at home. They have many areas where families can be together-and lots of activities for residents to choose from.

I am a CNA and had a client at Oakwood Manor in LaPorte, IN. The first day I went in it was very welcoming. The staff are wonderful, caring, and do alot for the residents. I can tell their hearts are for the residents and the residents are happy. I would recommend this place to anyone who is looking to live somewhere that they felt comfortable. Hands down this is where you want to go 💜

I experienced direct emotional abuse and was accused of committing a serious HIPAA violation by the Medical Records person, Gwendolyn. I experienced being yelled at, demeaning accusations, mockery, and outright lied on and falsely accused by the Medical Records Person, Gwendolyn. Lucky for me I have the audio of the entire incident otherwise It would be my word against hers.<br>This ruthless behavior by Gwendolyn occurred in her office, and she then instructed me to follow her to the Administrator’s office. While in the hallway, Gwendolyn loudly made comments including but not limited to:<br>1. They got the right one in this position today!<br>2. I bet you not getting those records today or any day!<br>3. You are not the responsible party for the deceased!<br>4. Your husband is not the POA for the patient!<br>FYI------The patient died on July 30, 2024.<br>I was requesting the Medical and financial records in November of 2025. POA ended when the patient died.<br>5. I was verbally reprimanded in the hallway, embarrassed, humiliated in front of other employees and family members. stating, “I know that you are not calling me a liar,” and “I know that you are not saying that I signed the HIPAA form.” However, I witnessed you sign it with my own eyes.<br><br>Once in Padra's office the behavior continued. Regrettably, Padra Francois did nothing to correct or cease this behavior that she observed from her employee in her presence. I am convinced that the elderly are not safe at Holly Hill Nursing Home. I do not feel that residents are protected by Management.<br>Gwendolyn’s incompetence is truly remarkable. She feigns ignorance of the fact that a power of attorney terminates when the person dies.<br><br>My husband and I decided to attempt to obtain the medical records again on a different day. Due to Gwendolyn’s previous inappropriate behavior, we contacted the Sulphur Police Department to meet us in the nursing home parking lot. The officer entered the nursing home and presented the Administrator Padra with the court documents that confirmed that the power of attorney had been revoked in 2019. In response, Padra informed the Police Officer that she was unsure what to do. filed a “No Trespassing” notice with the police for both my wife and myself, claiming that “We were causing problems and wanting my mother’s property.” Needless to say we were mortified by this unexpected, unjustified accusation.<br><br>We thought to ourselves that we would try one last time to get our Mom's medical records from the person who is actually calling the shots! Nomica Guillory. We were not gonna be intimidated by her false police reports. Sulphur police attempted to obtain the medical records, Nomica Guillory refused to speak to us. We have retained an Attorney.<br><br>It is crucial to be aware of your rights. If you believe that your loved one has been neglected, do not hesitate to contact unbudsmon.<br><br>When requesting records, it is imperative to demand them in writing. The relevant authorities have a 15-day timeframe to provide you with the requested records. However, the maximum time limit for providing these records is 30 days.<br><br>Finally, it is crucial to prioritize your safety and consistently record all conversations. In my experience, The Administrator Nomica Guillory has deliberately fabricated numerous false statements about my husband and me solely to prevent us from obtaining medical records that “he” is entitled to. She falsely claims to be unable to distinguish between an irrevocable trust, a power of attorney, and an estate without a will.<br><br>Consequently, when these false police reports are made, which damage your reputation, you can substantiate the falsehoods and seek appropriate relief.<br><br>Administration is a joke! She pays the hard workers barely minimum wage!! yet she drives around in a luxury BMW x 6. She is horrible

I'm a resident of crystal pines in crystal lake Illinois. I have been here for 8 months .I would like to give kudos to your staff for giving me a life back that I never thought I'd ever see again. I owe your people a debt that I can never repay. From your administrator Jeremy Kantor who is a loving caring human being to the whole staff of medical people and even house keeping and kitchen staff. Even the maintenance staff has gone above and beyond for me. If I wasn't retired I'd be honored to work with each and everyone of them. If I sound over the top emotionally, I am.
