Formerly known as Legacy Health and Rehabilitation Center, services at this facility initially degraded after new ownership took over in June of 2026. The new phone system worsened the problem they already had of not being able to reach staff, with disconnections being the new norm until mid July 2026. For weeks, even when they did answer the phone, they were likely to put a caller asking to reach the nursing or other staff on hold indefinitely, even when requested to watch the phone to see if anyone has picked up. By late July, 2026, their phones were at least being answered without callers being disconnected. They have had difficulty getting patients to outside appointments under previous management, so it remains to be seen if they have improved their transportation protocols. They sometimes serve poor evening meal selections during holidays and weekends, so perhaps new ownership will improve that. As of late July, 2026, they still haven't set up a secure web portal for residents and their representatives to transmit sensitive information, or if available, notification of such has not been sent out. Also as of late July 2026, they sometimes misplace laundry for residents even though the articles are labeled with the residents' names.
