Unsafe discharge practices and severe neglect. From day one, this facility prioritized insurance company cost-cutting over patient recovery. Instead of focusing on rehabilitation, administration harassed us daily with aggressive phone calls demanding discharge before it was medically safe.The actual medical and rehabilitative care is sub-standard. Patients are offered minimal therapy sessions, far below what is required for recovery. Furthermore, basic needs are ignored: the food is borderline inedible and general staff service is incredibly poor. They operate like a clearinghouse to save insurance companies money rather than a healthcare facility meant to make people well. Avoid this place at all costs; it needs a formal state investigation.

I wish there was a way to give negative ratings. If someone you love is considering this place, do whatever it take to change their mind. They will thank you later. I promise you that this is the worst available option.

⸻ My mother was a resident at this facility for only two weeks, and during that time she fell three separate times. After the third fall, we removed her the very next day due to serious concerns for her safety. The overall condition of the facility was also very concerning. It was uninviting and had a persistent unpleasant odor. Even more troubling was that we did not feel my mother was treated with the dignity and respect she deserved. This experience raised significant concerns about both the level of care and the environment. Based on what we observed in a very short period of time, I would strongly caution anyone considering placing a loved one here to look carefully at other options.

I was in the Rivers for 12 days following a total knee replacement for rehab. I was rather nervous because I had never had this type of care before and boy was it painful. Everyone took amazing care of me with kindness and empathy, while all were good there are a few I want to acknowledge for going above and beyond. Lanika Jones, Nurse Brown ❤️ Tanika (who knew I loved the rolls) Aisha who kept my room spotless and smelling wonderful Nurse Maja, Nurse Jan and Brittany Brown. Danille in PT who worked me out and gave me encouragement when i had my moments and got my ice packs ready and Altheaa who worked my arms to gain strength to master my walker and cane. These ladies looked in on me all the time and made me so comfortable. I was able to be discharged home yesterday the 27th (whole other story lol). Thank you again to these special ladies!!!! Warning: This review applies only to The Rivers Physical Therapy (NOT their Independent Living, which is excellent). If you love your family member, DO NOT let them stay here. The food is miserable, and the care is worse. My mom was left for hours when she needed cleaning, which caused a severe rash. By the end of her two-week stay, she looked so neglected that I wanted to cry — she left in far worse shape than when she entered. Yes, there may be a few caring staff members, but overall the culture felt dismissive and bare minimum. Most of my requests were ignored. It was heartbreaking to watch. The day after leaving, my mom already looked 1000x better. That says it all. This facility is, in my view, a dangerous place for vulnerable people. I will never allow my mom or anyone I care about to set foot there again.
