Our family’s experience at Pathways was deeply disappointing and heartbreaking. We entrusted the facility with our father’s care during one of the most vulnerable periods of his life, but the quality of care declined to the point that we no longer trusted Pathways to keep him safe.<br><br>This review is based on what our family personally witnessed over a year through frequent visits, communication with staff and leadership, and a Ring camera installed in our father’s room.<br><br>When our father first entered Pathways, the facility was led by Eileen. In our experience, the environment was compassionate, engaged, and resident-focused. Staff were attentive, residents participated in meaningful activities, and communication with families was strong.<br><br>After Eileen was let go and Sis Wichterman became Administrator, the culture changed dramatically. Many experienced nurses and caregivers left, activities declined, and staffing and supervision was increasingly inadequate. Families were not proactively informed about these changes.<br><br>Through the Ring camera, we witnessed consistent extended periods when no staff checked on our father. He wandered alone despite requiring a walker, entered other residents’ rooms without intervention, and spent hours without meaningful interaction. He experienced multiple falls, including one we witnessed on camera, after which he struggled without timely assistance.<br><br>We repeatedly had to advocate for basic medical attention. Changes in his condition did not receive appropriate urgency, communication was inconsistent, and concerns involving catheter care, medication administration, and documentation were not always addressed promptly. Nursing staff often appeared stretched too thin to provide timely assessment and intervention.<br><br>We were told the memory care wing would have approximately 20 residents with four caregivers and appropriate nursing coverage. That level of staffing was not maintained. Basic needs, supervision, and meaningful resident engagement were not reliably provided.<br><br>When serious concerns arose, leadership was difficult to reach, rarely visible, and largely absent during critical moments. We experienced very little transparency or accountability.<br><br>Anyone considering Pathways should ask detailed questions about staffing ratios, caregiver turnover, licensed nursing coverage, fall prevention, medical oversight, activities, and leadership visibility. Visit at different times of day and do not rely solely on the admissions presentation.<br><br>No family should need a camera to determine whether a loved one is safe. We installed one, and it documented prolonged isolation, unsafe wandering, inadequate supervision, and multiple falls.<br><br>Based on what we personally witnessed, we intend to report our concerns to the appropriate Pennsylvania authorities and request an independent review. We sincerely hope meaningful changes are made so no other resident or family experiences what ours did.
