Professional Services Care

Our trained and certified staff is available 24 hours a day, seven days a week and led by full-time, licensed nurses. Our staff receives constant supervision from our physician partners who also perform routine resident visits. Surpass has equipped each residence with a state-of-the-art emergency call system.

Respite Care

Respite Care equals less than a 30-day short-term stay at The Heritage at Hunters Chase. Primary caregivers can use this program to take a recharging break for a few days, or family members can use it as an assisted living trial period. All respite care residents have access to all the services and programs our communities have to offer, including medication management, daily living assistance, health monitoring, meals and refreshments, and our Life Engagement program.

Day Respite Care

Surpass Senior Living’s day respite program allows primary caregivers to take some time during the day to care for themselves and their busy schedules. Our care partners will involve your family member in community life. Meals, snacks, and refreshments are provided throughout the day.

Meaningful Life Engagement

The Heritage at Twin Creeks’s state-of-the-art memory care program delivers expert assistance to those affected by memory afflictions.

The Traditions Program

The Heritage at Twin Creeks does more than care for your family member’s basic needs. We provide our residents with amenities designed for their comfort. The Traditions' memory care program at The Heritage at Twin Creeks incorporates a comprehensive, meaningful life engagement program that aims to lower anxiety, decrease frustration, and increase feelings of independence, fostering a sense of purpose.

All of our professional staff, nursing personnel, and Resident Care Partners receive proprietary training in Alzheimer's and dementia-related care. Surpass Senior Living’s Resident Care Partners have met or exceeded state-required certification and participate in continuing education.
