This facility is a disappointment. Disorganized, communication from staff was very inconsistent. Response to concerns was very delayed. I would recommend families to visit in person before making a decision to have their loved ones stay here.

We moved my mom, Laurice, to the guardian facility, and I was initially skeptical. However, after a few months of being actively involved in her stay, I’ve come to realize that it’s a fantastic place. The nurses and nurse aides are incredibly caring, loving, and always willing to help patients. Paul, the nurse, is particularly kind and attentive. Helen, another nurse, is incredibly kind and does her job exceptionally well. Belinda, the CNA, tries to keep things light-hearted and humorous while she works. Linda, another CNA, is amazing and loving. Lucile, the CNA, is very helpful. From top to bottom, Nancy, the director, and all the nurses are great. The facility is clean, and there are many activities offered. The food is acceptable, but I’m sure they’ll make improvements.

My husband has been at The Guardian Center for several months since his stroke. They installed a phone for him when I initially requested it but it broke about a week later. They replaced it but this time didn't give him his own number, it just comes up the main number. So for the past 3 weeks I have been unable to reach him by phone. I cannot call to check on him when he's ill, or to say goodnight every night. He is lonely and miserable. I have called every single day for 2 weeks about fixing his phone line. The 2nd floor unit manager never answers her phone or returns my calls and nothing has been done to fix the situation. My husband and his roommate also have not had a functioning clock in over a month and have no idea what time it is at any point of the day. I have been calling and complaining about that for over a month! The aides are not very friendly and for some reason they lied to the nurse on duty and accused us of smoking in his room! We don't even smoke! This place does not care about its residents or their families. No one on the 2nd floor answers the phone after 7 pm so good luck checking on your loved ones at night if they don't have their own phone! Zero stars. Do not recommend.

I was recently at Guardian visiting a cousin. I was thrilled with her progress and recovery. The staff was extremely helpful and friendly. Nancy, the administrator, was kind and caring. She was very understanding regarding our concern and reassured us in no time. She seems to sincerely care about all the residents there. I am glad that my cousin is in such good hands.

I was sent to Guardian Health on Main Street, Brockton Mass after two broken shoulders with surgery on one. I arrived 24 hours after recovery. Ambulance driver asked if I knew where I was going? First Red Light!! I was delivered to 3rd floor. Not one person spoke English. This was 4:00 pm on a Friday afternoon. They plopped a tray in front of me shortly after. It was a 10 cent hamburger roll with a tablespoon of Sloppy Joe mix and a side of dead cold , dried out Lima beans. Nasty old dirty coffee that was room temp. I could not feed myself with two broken shoulders. I asked for help. Not a chance!! No one would help me get to bathroom, so they decided to put me in diaper and leave me alone. Creepy weird to say the least. Dinner at 4:00 pm ish, lights out at 7:00 pm. I was paired ( shared room) with a mentally handicapped youngish woman who had to watch cartoons very loud all night. Then a woman showed up at 7:30 ish pm whom wanted to perform a TB test. She was yielding a syringe in the dark. I told her absolutely Not !Needless to say I voice called my daughter at 5:00 am to get me out or I’m calling the police. It took almost 3 hours to get out with out violating AMA. I’m really sorry that these places exist. It’s just a place where People go to die. Be very careful of your Medicare coverage. It’s really frightening what’s out there.
