As a patient at Five Towns Premier I must say I had a wonderful experience. All good things I must say from the administrator Avi and Penina to the concierge Malisha the nursing team on the 3rd floor department was very helpful. Physically therapist got me back up on my feet I couldn’t say enough good things about this place. Great experience…

No one wants to be in rehab. I get that , but if you are in that situation,the place to be is here. The positve atmosphere must start at the top and work its way down. Mr. Yitzchok Wiener the CEO runs this place as if his own family is here. The truth is he feels everone here is his family The administrator Avi , a soft spoken individual is proof positive that you dont need to be a harsh fellow to get things done. His genteel approach is what this place thrives on. Kudos to him. As far as the Medical team goes the Doctors the wound care nurses and the ones that administer the pharmaceuticals are 👍 David the Nurse Supervisor ,Ms. Greene Ms. Stickney Ramona,Christina,James and others (please forgive me in terrible with names) The Nurse aides are spectacular Special! Shout out to Shelly. All of you are great and you should be proud of what you do. Every good organization has a person that has a specific title but whose duties encompass a whole lot more than that title. That person is Malisha. She just about anticipates and covers every conceivable problem that you might have and makes sure you get a super positive response. Thanks to all of you As far as Rehab goes Eli , Savita , Frieda ,Alex, Harry and al the others 👍👍👍👍👍👍 Sincerely and with good wishes to all Aaron S

They don’t just plan activities. They create moments. They make residents smile, laugh, feel included, and feel remembered. Whether it’s holiday events, games, music, intergenerational programs, or just taking the time to truly connect with people, you can feel how much heart they put into everything they do. I’ve seen firsthand how hard Anthony works to bring energy, kindness, and creativity into the building every day. He treats residents like family and always goes above and beyond to make things special not because he has to, but because he genuinely cares. The Recreation Department has helped create memories that residents, families, and volunteers will never forget. Thank you for all you do for the community and for making such a positive difference in so many lives. Five Towns Premier is lucky to have you.

I am writing this review as the son of a resident who recently spent approximately eight months at The Five Towns Premier Nursing and Rehabilitation Center. Choosing a facility for a loved one is one of the most difficult decisions a family can make, but looking back, I know we made the right choice. The highest praise I can give is this: when it was finally time for my mother to return home, she actually didn't want to leave. During her eight months there, the facility became much more than just a rehabilitation center to her—it became home. She frequently commented on how attentive and kind the staff were. Whether it was the nursing team, the therapists, or the support staff, she felt truly seen and cared for as an individual, not just a patient. She felt safe, respected, and genuinely cared for. Even now that she is back home, she talks about how much she misses the people who worked there and the daily interactions she had with them. To the team at Five Towns Premier: thank you for treating my mother like family. Your dedication made a difficult transition in her life a joyful and comfortable experience. I would highly recommend this facility to any family looking for a place where their loved one will be treated with the utmost dignity and warmth.

First, let me say that the facility is beautiful and clean. All of the workers there are from the nurses to the aides to the Cleaners to the helpers, Are all wonderful. You are greeted by a beautiful woman named Rosemary. Always has a smile and gives me great vibes. My dad went there after he fell broke his hip. After being discharged he was brought to this facility. He was comfortable. They helped him a lot. The aides are amazing!!! Brought him to bingo and painting. My dad has dementia and also the language barrier is very difficult. The doctor was amazing too. I can only speak of the 2nd floor. I cannot express enough how amazing everyone is!!! I cannot pin point each person because they are all comforting and patient. Highly recommend this facility !!!
