Please avoid this facility at all costs!<br><br>What's wrong: Communication and accountability amongst the professionals from the top on down.<br><br>There is no Maintenance personnel at all! Many things are broken including the hot water(either none at all or scolding hot depends on day) & water pressure. Beds, hoyer lifts and sit-to-stand lifts are malfunctioning.<br><br>A/C just started working (6/5) but only on one hallway.<br><br>State is consistently in the building investigating resident complaints yet management never has an action plan to resolve issues (residents not getting showers, meals are served cold all the time, short staffing on 2nd shift everyday). There was even an instance where unauthorized "caregivers" were providing care to residents (not an employee nor agency staff member).<br><br>Many wound care concerns & guess what? No wound care nurse nor infection control nurse! Therefore they are unable to provide the skilled nursing care that they advertise for.<br><br>Their "sister facility" about half a mile down the road gets better reviews than this one!<br><br>The good: No rodents or insects.

I have been there a few times visiting a friend and when I have signed her out the women at the front desk were rude. They were all engulfed in their phones to give me the sign in/out book they told me to do it myself. They couldn't got off their phones to give me the book or show me where to sign at. Pretty pathetic
