This place will kill your family members!<br><br>I have serious concerns regarding the care my mother received while a resident at St. Sophia Health & Rehabilitation Center.<br><br>During her stay, my mother experienced what our family believes was severe neglect and a failure to protect her rights as a patient.<br><br>* Neither my mother nor her family were adequately informed of significant changes in her care plan, including decisions that could have financial implications.<br><br>* Despite having insurance coverage for rehabilitative services, she did not receive the physical therapy and occupational therapy that were expected as part of her treatment plan.<br><br>* Her Foley catheter reportedly went unchanged and was not properly monitored. As a result, it became clogged, leading to approximately 2,000 milliliters of retained urine and requiring emergency medical intervention.<br><br>* While bedridden, she was not repositioned appropriately, resulting in severe pressure ulcers (bed sores).<br><br>* Communication from the medical team--including nurses, nursing assistants, physicians, and administration--was extremely limited, leaving our family uninformed about her condition and treatment.<br><br>* She developed a serious urinary tract infection that ultimately required hospitalization.<br><br>* She also developed pneumonia while residing at the facility.<br><br>*And that's just this month. She has had more issues since her stay started March.<br><br>We believe these events reflect significant failures in nursing care, medical oversight, infection prevention, patient advocacy, and communication with both the resident and family members.<br><br>BEWARE!

My loved one is currently here. Someone told him he pushes his button too much. All he asks for is water .. food. To use restroom as he's on hospice with a couple weeks to live. They wasn't putting him on the toilet. Causing a panic attack. When the ambulance came with a police officer ... the door was short and his air had been turned off and he was pouring sweat. This is the 4th time they have done this. He's bed ridden. He wasnt get fed all day sometimes. Well till late. I finally put a camera in his room. Big difference. Except today they brought him man potatoes

Customer service is terrible been trying to get a hold of H.R. no answer call the administrator no answer called the front answer they had nothing but attitude didn't help one bit transfers me to a office with no one in it all I'm trying to do is get my W2 before its to late.

If I could I would give them zero stars. My mom went in on 3/27 and I removed her on 3/28. The room was filthy and the bathroom was filthy and smelled of urine. The dinner that was offered to my mom was what they served for lunch. The following day my mother didn’t receive her morning medicine until 10:30, and they never served her breakfast or lunch. The administrator needs to be replaced and staff retrained. This place is not fit for humans!!!!!

I would have to agree with Nathan the review below my loved one was here as well . They did not take care of my loved. One was whatsoever They left them to die. they sit at the front nurses station and just watching love ones come in and out sitting there in their scrubs feeling high mighty, but I can bet you one thing what goes around comes around someone purposely did this to my loved one because they was tired of caring for them . They saw my love one struggling that day and needed help. I bet you they walked away and took their time to wait on Them to die if you did not want to be a loving caring nurse And have love and compassion for people and their loved ones why become a nurse so kill people that you’re tired of taking care of. Counter days because we all have a day.
