This place will kill your family members!

I have serious concerns regarding the care my mother received while a resident at St. Sophia Health & Rehabilitation Center.

During her stay, my mother experienced what our family believes was severe neglect and a failure to protect her rights as a patient.

* Neither my mother nor her family were adequately informed of significant changes in her care plan, including decisions that could have financial implications.

* Despite having insurance coverage for rehabilitative services, she did not receive the physical therapy and occupational therapy that were expected as part of her treatment plan.

* Her Foley catheter reportedly went unchanged and was not properly monitored. As a result, it became clogged, leading to approximately 2,000 milliliters of retained urine and requiring emergency medical intervention.

* While bedridden, she was not repositioned appropriately, resulting in severe pressure ulcers (bed sores).

* Communication from the medical team--including nurses, nursing assistants, physicians, and administration--was extremely limited, leaving our family uninformed about her condition and treatment.

* She developed a serious urinary tract infection that ultimately required hospitalization.

* She also developed pneumonia while residing at the facility.

*And that's just this month. She has had more issues since her stay started March.

We believe these events reflect significant failures in nursing care, medical oversight, infection prevention, patient advocacy, and communication with both the resident and family members.

BEWARE!
