About St Luke's Helen G Nassif Transitional Care Center

St. Luke's Helen G. Nassif Transitional Care Center (TCC) in Cedar Rapids is a recuperative and supportive setting designed to help individuals heal following an injury, surgery or serious illness. It bridges the gap between a hospital stay and returning to a long-term home, offering short-stay rehabilitation as well as enhanced care and long-term care options for those who need extra time or ongoing support.

Care is provided by a medical director working alongside nurse practitioners with expertise in geriatric post-hospital care, who are on-site daily. The broader care team includes nurses, social workers, and physical, occupational and speech therapists who develop individualized daily therapy sessions and personalized care plans in coordination with hospital and primary care physicians. Teams meet with patients at the bedside early in their stay and weekly thereafter to review progress and set recovery goals.

Enhanced care allows individuals additional time before transitioning home after a skilled stay, combining short- and long-term personal and health care services. Long-term care is available for residents who are unable to return home, with programming focused on health and social wellness and daily staff interaction to support residents' needs.

Amenities include an enhanced therapy gym equipped with rehabilitative technology, a salon space available for a beautician or barber, spacious private rooms with a zero-entry shower and bench seat, and rooms furnished with TV, basic cable and Wi-Fi. The facility features large open hallways with social areas for mingling and outdoor sitting areas for warmer days. Admission transportation is available free of charge from Cedar Rapids hospitals to the center, and patients and families can track daily schedules through a web application or on paper.

Family and friends are encouraged to be involved in the care process, including meeting with nurse practitioners and nursing staff to stay informed on care plans, and guests are welcome at any time. The center has been recognized as a “High Performing” skilled nursing facility, placing it among the top 19 percent of SNFs nationwide based on an evaluation of publicly available performance data.

I love this facility! It is very clean and doesn't smell like a nursing home. I also see where the DON and the ADON are on the floor helping the aides and family. Everyone here is GREAT! Thank you for all you do for the patients and families
