St. Luke's Helen G. Nassif Transitional Care Center (TCC) in Cedar Rapids is a recuperative and supportive setting designed to help individuals heal following an injury, surgery or serious illness. The broader care team includes nurses, social workers, and physical, occupational and speech therapists who develop individualized daily therapy sessions and personalized care plans in coordination with hospital and primary care physicians. Teams meet with patients at the bedside early in their stay and weekly thereafter to review progress and set recovery goals.

Enhanced care allows individuals additional time before transitioning home after a skilled stay, combining short- and long-term personal and health care services. Long-term care is available for residents who are unable to return home, with programming focused on health and social wellness and daily staff interaction to support residents' needs.
