A very nice nursing and rehab center. Clean and cheerful. There are assisted living apartments next door, too.

My mother has been a resident for the last year at St. Croix Health and Rehab. The facility is beautiful with large private rooms for each resident. The staff have been AMAZING and provide the best care and support possible. I feel fortunate and can rest easy knowing my mother is well taken care of. Highly recommend to anyone looking for a long term care facility for a loved one in St. Croix County area.

This facility is very clean and well kept. The residents are greatly cared for and the staff is well qualified and thorough in the care provided. The rehab team is wonderful and the Long Term Care rooms are spacious and set up well. If you have need of a place to stay for rehab post surgery/ hospitalization or as a long term option, I would strongly recommend St. Croix Health Center!

Our family member is moving from a group home in MN to the St. Croix Health Center. Trying to navigate changing her services and SSI from MN to WI on our own was stressful and daunting.
We met with staff today that walked us through everything that needs to be done. We left with a list of 3 tasks to take care of and a tremendously lower stress level. They are taking care of the rest. Needless to say, we are thrilled with the staff!
The facility is in pristine condition. There is a full range of PT equipment, billiards, planned activities, and a salon. All rooms are private, adequately sized, and able to be personalized with a built-in knickknack shelf and the ability to put up pictures on the wall. Each room has a private bathroom with wheelchair accessibility, handheld shower, and built in seat in the shower area.
Not only are we relieved to have found this facility, but we are excited for our loved one to be moving here.
