This is the 2 nd time my mom has been in this facility.. the Staff has been extremely supportive. And she says they are wonderful. I do wish there was something that could help improve the facility its self . While the staff is amazing they to deserve better . The AC was not working well for the sweet lady at the front desk tv's not working bathroom doors about to fall off .. there's got to be help from the local community. I wish I could just get a list of the needs and wants from the place and try to find them some community help . Thank you so much for taking care of my mom . I want to help you help others ..

This facility performed a miracle for my wife after a 6-week hospital stay left her bedridden, unable to swallow, and requiring total care. Thanks to 2.5 months of expert rehab, she left feeding herself, with her PEG tube scheduled for removal, full bowel function restored, and the ability to walk 165 feet to the bathroom with a walker.Special thanks to her phenomenal therapists—Sara, Simone, Sidney, Laura, and Lauren—who truly cared about her success and said it was the best comeback they’d ever seen. Sara even brought flowers and cake for a beautiful send-off celebration photo.The entire staff here is like a family and truly care. Also thanks to Social Services director Jasmine, Financial Dept Blake, Admission Sonya and CNA Ashley. The Therapists, Nurses and CNAs here are outstanding, compassionate professionals!

My wife’s sister just completed a 4 month stay at St Augustine Health & Rehab. We were very happy with the kind, considerate care she received. The staff was extremely helpful and attentive reflecting the administration’s dedication to the patient’s wellbeing.

My father, Frank, was here for a couple weeks for PT. The staff is 98%super and does a great job with no issues. Kudos to Mia, Glenda, Melissa,Noah, Simone, Sara, Tarnisha, Kim, and many more. The food is acceptable, most meats are ground up and so not too great, the rest is good. Alternatives are available on request and include grilled cheese, hamburgers, chef salad, and are really well done. Thumbs up on the grilled cheese!! It’s an older building but adequate. Therapy is very well done. 24/7 controlled entrances and exits provide security. My father cut his leg severely climbing into my truck, and staff was on it in seconds stopping the bleeding and bandaging. It was not at all the facility responsibility for the injury, not at all. Went to ER and 19 stitches later he is on the mend. The staff has done the wound care follow up cheerfully and professionally. Many thanks for all the great treatment and attitude in every situation!!

I had to place a star in order to make a review - otherwise it would have a no star review. I had my mother there THINKING it was a good place since it is near my home. It actually was the worse place and I would never recommend. My mother had a single room and had an emergency while there that she had to be seen at Flagler Hospital. She was gone for a couple of hours and when we got back they placed another person in her room - my mother’s belongings were packed at the door and several of her items were stolen. I had to go to laundry room to where none was found. When I went the next day to make aware to the manager she wasn’t listening and just kept eating her lunch with her husband as if she could care less. Also the nurse that past meds didn’t wear gloves and was handling medication in an unsanitary way. Not once did the manager help me find or approach the missing (expensive) clothing my mother lost
