Awful place. Do not send your loved one here! My dad was there when it was Autumn Leaves and then Liana. I won't go into the details, but they were horrible with care 2018-2019, right after we installed cameras.

Management is rarely onsite. They like to hide out in their offices. They are always understaffed with caregivers. If you have a family member living here whose dementia is mid-advanced, you'll need to hire someone to sit with them and make sure the basics such as changing linens, showers, encouraging hydration, giving meds in a consistent and timely manner are done or you'll need to check on these things every day. There are a few staff members really trying, but without good leadership and managers, they're in an uphill battle. I would not recommend this location based on what we've experienced since our family member has lived there. The food has improved in the past few weeks as they hired someone who really seems to understand who is eating the meals and trying to make it better. Looks like a new facility with great decorating if that's all you want for your $$$$.

Time for an update... Turns out it was a budget issue. Every slip I have noticed is all about the lack of money. Autumn Leaves is now in bankruptcy. According to administrators "all will be fine". I think that is what you are supposed to say in this business. However the Director is leaving, the nurse just made a transfer to a new location so we are now on our 4th nurse in 2 years, and as of today I hear the Assistant Director is leaving!!!! A full time member of the cleaning crew just quit. And to top it off the food is still mediocre. The building is bringing in residents as fast as they can and are not equipped for many who are extremely high need. Our Assistant Director is one of the most caring, big hearted staff members we have ever had in leadership during the 2 years we have been part of Autumn Leaves. Her absence will be enormous in the current climate. She is the ONLY full time administrator on property. Now we wait for the next shoe to drop. Right now it feels like we are raining shoes. I could not go on in good conscience without saying Do Not Bring a Loved One Here Right Now. Don't do it!! Wait to see if they can right the ship.

Who in their right minds, no pun intended feeds whole baby carrots in left over spaghetti to dementia patients when some can't even cut their food or negotiate utensils. They should be eating 5 star food not this. Their water, tea glasses should be kept filled with encouragement to drink as well but it doesn't happen.

Well, we had Mom at A.L. for 8 months. She needed hospice care in the end and A.L. was able to accommodate that. Mom got to be in her room with the same aides who cared for her so well all along. Prior to the very end, she LOVED being here. She became quite the social butterfly and helped new residents learn the ropes. It was our first venture into memory care and Autumn Leaves made a difficult time go smoothly. The director, activities staff and nursing staff do a wonderful job of getting mom into doing things that she enjoys so that she feels like she is contributing. The feeling is warm and welcoming like your own home without being sterile or like a "nursing home". Autumn Leaves places a priority on cleanliness and safety.
