Senior LIFE Washington

Senior LIFE Washington is a care coordination program in Washington, Pennsylvania, designed for adults age 55 and older who qualify for Medicaid. The program enables participants to receive comprehensive care while living safely and independently at home for as long as possible.

Through Senior LIFE, participants access a variety of services including rehabilitation and other medical care coordinated by a team of healthcare professionals who specialize in geriatric care. The team includes doctors, nurses, specialists, physical therapists, home care aides, and other professionals dedicated to providing individualized care based on each participant's specific needs. Services are available 24 hours a day, 365 days a year.

For those who qualify for Medicaid and reside in the Senior LIFE coverage area, there is no cost to join the program. Participants must meet the level of care determined by the State and be able to safely live within the community. The LIFE Health and Wellness Center operates Monday through Friday from 8 am to 4:30 pm.

Senior LIFE serves residents across a broad coverage area encompassing numerous zip codes in the Washington, Pennsylvania region. Most services must be preapproved by the interdisciplinary care team, though in-network and out-of-network emergency services are covered. The program is voluntary and participants may disenroll at any time.
