Senior LIFE Indiana is a Medicaid-based care coordination program located in Indiana, Pennsylvania, designed for adults age 55 and older who qualify for the program. For those who meet eligibility requirements and reside in the Senior LIFE coverage area, enrollment is offered at no cost.
The program provides comprehensive healthcare services coordinated through an interdisciplinary team approach. Services include rehabilitation, physical therapy, and preventive care, available 24 hours a day, 365 days a year. Senior LIFE's healthcare professionals specialize in geriatric care and include doctors, nurses, specialists, physical therapists, home care aides, and other trained staff dedicated to meeting each participant's individual needs.
The program enables participants to receive care while living safely and independently at home for as long as possible. All services except emergency care must be preauthorized by the interdisciplinary team. Emergency services, both in-network and out-of-network, are covered by Senior LIFE. The LIFE Health and Wellness Center, where participants can access services, operates Monday through Friday from 8 am to 4:30 pm.
To qualify for Senior LIFE, applicants must be age 55 or older, meet the level of care determined by the State, live within the Senior LIFE coverage area, and be able to safely live within the community. Senior LIFE is a voluntary program, and participants may disenroll at any time.
