Great rehab facility!! Staff and workers were all very friendly, helpful and did great job on my husband’s therapy. Really nice size private room with a window. Facility is kept very clean. The food was very good also. Would highly recommend.

They will be kept alive and with “food”. The compassionate care is not there with most of the staff - I 2nd and have seen same symptoms - it is mostly related to the administrative and ownership greed ! But this is accurate below from what I’ve experienced so far - we saw the same with blood sugar management ! Sharon Casey Local Guide·20 reviews·1 photo Edited 2 years ago This facility is awful. Complete negligence from the beginning. I took my husband out of there after 2 hours. No one checked him and his oxygen level went dangerously low. He arrived via transport from the hospital. If he had not figured out how to call me, we could have had a much different outcome. His nurse admitted after an hour and a half of being there that NO ONE even read his chart or looked at him. He requires oxygen! I would not board my dog there. I only gave one star as there was nothing less.

Scott Lake Health & Rehab is one of the best facilities in Polk County. My mother has been here twice for physical rehab and now is part of their long term care unit. It's an extremely clean facility and the staff goes out of their way to care for their patients and residents. Special shout out to the nurses, who truly care, including Charity, Kelly, Trisha, Bella and Tiffany (nurse manager). And, CNAs Ellen, Narda (as well as many more). The Physical Therapy staff here is wonderful and really helped my mother gain some of her strength back. We will forever be grateful to Sonya and Conn. They have plenty of daily activities to stay social and engaged with others. My mother has positive feedback on the quality of the food and appreciates there are alternative options if they don't want what is on the main menu. This was a difficult and emotional process for our family to navigate and I appreciate how the admin staff dedicated the time to helping my mother and our family adjust to the transition to long term care. Thank you to Michelle in Social Services and Ivelice in the Business Office for all you did. I would highly recommend Scott Lake Health & Rehab if you're seeking a quality facility to care for your loved one.

My husband had excellent care while at Scott Lake. The facility is kept sparkling clean, the nurses and cna’s are very pleasant and extremely dedicated. There are many activities for patients. Christmas decorations went above and beyond festive which made the holidays more palatable. The PT/OT/Speech staff care deeply about their patients and the results speak for themselves. The receptionists are all super friendly and welcoming 😊 Our social services director Michelle made sure everything was in order upon discharge for a smooth transition to home. ⭐️⭐️⭐️⭐️⭐️
