One Heartland at San Antonio Wellness and Rehabilitation is a skilled nursing and long-term care facility in San Antonio, Texas, licensed for 154 residents.

Family experiences at the community vary noticeably. Several families praise the nursing staff by name, noting their attentiveness and patience, and describe the facility as clean with a friendly atmosphere and regular activities. One family wrote, "They all take really good care of my mother." However, dietary service has been cited as inconsistent, with at least one family reporting repeated meal errors and a manager's rudeness. My family member was here and was viciously attacked by three Brown recluse spider bites, and they suffered two terrible falls from bed! And yet, after all this chaos and pain, I am still waiting—no answer, no explanation, no accountability for how this could possibly have happened! It’s beyond frustrating—completely unacceptable! I had no choice but to move my family member to protect them because the level of neglect and carelessness is infuriating! The medical staff appears to be uncertain about the most appropriate method of providing assistance or may lack sufficient training in diagnosing.

This is my second time going there I can honestly say is ok facility the only reason I don't give them 5 stars is because of The Dietary Manager he was rude never got my food right...... But the nurses like Elsa and Rosa are great also I would like to say thank you to miss G she was always very nice and patience because I have strong personality .
