About Pruitthealth - Washington

Pruitthealth - Washington is a 47-bed skilled nursing and long-term care facility in Washington, Georgia, serving patients recovering from acute illness, stroke, and other conditions requiring rehabilitation and ongoing medical support.

Family experiences at the facility vary noticeably. Positive reviewers consistently praise staff warmth and attentiveness, noting that residents appear clean, well-cared-for, and at ease during frequent visits. One family wrote that staff "are always courteous, kind, respectful and friendly." Several families highlighted specific nurses and care coordinators by name for their responsiveness and compassion. On the other hand, some families report significant gaps in care delivery: instances of prolonged waits for bathroom assistance, missed room checks during visits, and inconsistent communication among staff members about dietary restrictions and medication protocols. One reviewer noted concerns about diet oversight, describing a resident placed on a pureed diet who received regular toast instead.

The facility earns a 4.4 out of 5 rating across 17 Google reviews. While top scores celebrate cleanliness and the welcome process for new residents, lower ratings point to inconsistency in day-to-day care quality and suggest that performance may have declined over time according to some longer-term families.

When we first decided to put our father there we considered things that would benefit his overall health. In the last year or so the care team has declined. The top leading staff has fallen off. He is caring a lot for himself except his meds. This place need a serious investigation with family members & residents.

We put our love one in there for therapy, all he did was lay in the bed in a paper for over a day. He waited a whole hour for someone to come so he could use the bathroom and that was a family member, no one came into the room the whole time the visitor was there. They tried to give him fish and he has gout. Was very angry when he was removed from the facility and was denied his medication. Never again will we put another person in their care.

I can say the staff was nice, but they all contradicted each other with everything. ( giving pills to discharge) Definitely not the cleanest place. She was suppose to be on a finely chopped diet and they gave her heel of the toast.SMH. Old very thinning sheets.

I love the way the staff cares for my Mom. They are always courteous, kind, respectful and friendly when I visit. I visit my Mom nearly every day and at various times throughout the morning, afternoon and evenings. During my visits my Mom is always clean, happy and cared for in excellent condition, the Facility is always clean inside and outside and makes you feel at home, and the Staff - everyone from the managers, supervisors, nurses, counselors, pastors and maintenance employees are supreme in the way they conduct themselves and treats My Mother and me. Recently one of my brothers have moved down and he is with me when I visit and he is treated with the same respect. I love the extra activities such as Exercise Class, Bingo, etc...Parties for every season and Holidays. The constant care around the clock. Also if I need to carry my Mom away from the Facility, they have her ready to go at time I requested. My Mom seems to be enjoying her self every time I visit. Thanks for being there for her. Recently, I have noticed that more help is needed on the weekends. I have bought this to there attention. When I visited last weekend, they seem to have improved the situation. Thank you for caring. Sincerely, Sylvester Frank Wright

From the moment we found out that my mom was going to be here for rehab from a stroke we were welcomed. Paperwork was simple and explained well. Julie Berry and Caroline Danner made the experience a breeze. When my mom arrived she was taken care of immediately and made to feel welcome. All of the staff has been great. Shelby Bailey quickly became mama’s favorite nurse and she is great! The meals have been so good. I would recommend heritage to anyone!
