New Great: Esther RN, Tory RN Jessica CNA., (all 3 were absolutely the best staff who took care of me. Very caring and compassionate) Michelle in Hospitality, very nice Great: Physical Therapy..Edwina, Matt, Occ therapy: Rhonda Rachel unit secretary was helpful and kind. There were some issues during my stay. 97% of the staff do not wear name badges and do not always introduce themselves. Most of them should not be working in that facility or any other for that matter. The white boards in my room did not always say the name of the staff taking care of you. It could be the wrong names for 2-3 days. 11/8 Never got my dinner tray from the aide. Had to DoorDash my dinner which I wasn’t planning to do. 11/7 female housekeeper took dirty linen in my room bagged it and put the bag on the floor in my room where I needed to walk with a walker. I almost tripped over the bag. She never came back for it so I had to put it in the hallway. 11/7 a female patient and a confused male patient came into my room about 7:30p. I told them to get out and I notified the staff. They should have 1:1 for patients like that. Nurse i (first letter of her name) emptied my drain from surgery I had into my used urinal with urine encased on the top! I made my doctors aware. Talk about laziness! Grievance filed. She never smiled and had a bad attitude. She should not be in the medical profession. She was on the 11p-7am shift. Complaint will be filled against her license. That’s endangering a patients safety and health. I put a sign on my door Please do not disturb patient 11p-6am. A female aide comes in 5:10am and yells “ hey how are you and turns on the lights”. Very rude. Does she know not how to read!! I told the supervisor to keep her out of my room. 10/26/2025 Another nurse (name unknown) kept me waiting 2 hours 45 mins for pain medication. When she finally came in I asked her why she made me wait so long. She said she had an admission and that was more important than me! I heard she was fired bc another patient complained about her as well. Food: Best if you bring food from home or buy it from somewhere. The only thing good was the coffee. Many things missing from the tray and had to keep calling the kitchen. One day they did not put dessert on the tray so I called and they said they will bring it right up. I waited 20 minutes and finally found it on top of the nurses medication cart by the nursing station. Does it hurt to walk down the hall to my room and give it to me?! Lazy people. They mess up the menu a lot. They gave out plastic ware a few times with a paper towel (kind you find in the bathroom) instead of regular silverware and a real napkin. Some food ok. I was given a broken toilet riser and kept telling the staff but nothing was done. The back of it was broken. I finally found a maintenance man who was kind enough to take it out of my room a few days before I left. since I refused to use it. It was in the bathroom 3 weeks! Towels and washcloths can be left on the floor in the bathroom after you use them before it’s picked up. I had it in my bathroom 3-4 days. I was told only CNAs pick them up not housekeeping. The furniture is old and low. I needed a cushion from the couch to put on the recliner to make it higher. This facility used to be Promedica and Powerback. Staff told me they used to get prime ribs for meals and other good food and $10.00 would be taken out of each paycheck and there used to be a restaurant on the main floor. After Covid that all changed. They changed vendors hence food is not great. Employees have to either bring food from home or DoorDash it. One morning about 6:30a I needed a medication. I went to the desk to get it and the nurse was dozing off. Sorry to wake you but I’d like my pill. I got it 10 minutes later. Notice bacon on the breakfast tray. I do not eat bacon or pork products. This happened 3 times and found this disrespectful. I left some messages for the DON Catherine (director of nursing) to please see me to talk about some issues. Totally ignored me and never got back to me...

Do you have loved ones that you want to put somewhere that looks amazing but has a horrendous nursing staff? Then this is the place for you! They signed out my relative's medications, but never gave them to her. She had an accident, but had to wait two days because they only give showers once a week. You can't find a CNA except for at the beginning and end of their shift. They let people roam the floors, to the point that when we were visiting, the nurse saw the other patient walk into my relative's room, didn't say anything, and watched a that other patient tried to lure my daughter out of the room. They didn't even bother with therapy for my relative to work on stairs. I'm fact, OT & PT were basically on a «First Come/First Served," basis. Looks like a phenomenal hotel, which it might as well be, except you'd get better service at a hotel.

Changed name to Autumn Lakes and it is totally horrible here! Everything from the awful food, to being short staffed, employees who don't care and are complaining too, to no blankets (yes, they run out), to not getting changed and sitting in a wet bed from 12-20 hours (health department was here about that) at least a half hour when you pull the cord to get help, and now I'm breaking out in a rash when I lay in the bed ( not sure why but they're advice is to sleep in a recliner). I have been here 4 times, first time about 9 years ago and it's gone downhill! I'll Never come back!
