About Portola Gardens

Portola Gardens is an assisted living community in San Francisco's Portola district, housed in a 1930s Colonial Revival building that has served the city's seniors for generations. The facility is a residential care for the elderly (RCFE) with shared rooms and shared baths designed to keep assisted living affordable for middle-income families who wish to remain in San Francisco rather than relocate to facilities in outlying areas.

The community offers assistance with daily living needs and has been noted for hospice care services. Staff are trained in dementia care, and the facility participates in San Francisco's Assisted Living Waiver program, providing a Medi-Cal pathway for residents who qualify, ensuring that staying in the city does not depend solely on private wealth.

The original columns and gardens remain from the building's historic construction, while the dining room, kitchen, and main common areas were renovated in 2018. Residents have access to seasonal plantings, walking paths, and shade areas throughout the garden, which is visible from nearly every common room. The neighborhood is described as quiet and safe, secluded from downtown.

Programming and daily life at Portola Gardens includes live music in the parlor, afternoon art activities, garden time, and the presence of a house dog. The community emphasizes personalized care plans built around each resident's needs, place preferences, and relationships with family and loved ones.

We chose Portola Gardens to care for our mom during her time in hospice, and overall, we’re grateful we found this facility when we needed it most. The staff showed genuine compassion, and the administrative team was consistently responsive and supportive. Our mom didn’t speak English, but the diverse staff was patient and respectful in their interactions with her, which meant a lot to us. The facility is quite busy, with different caregivers working across various shifts. Because of this, we found it necessary to communicate our mom’s needs multiple times to ensure continuity of care. At times, it was concerning that some team members weren’t initially aware of her fracture, but once we brought it to their attention, they made an effort to relay the information to others on the team. Despite some communication challenges, we appreciate the kindness and dedication of the staff. Their care helped bring comfort to our mom and to us during a very difficult time.

Portola Gardens Assisted Living in San Francisco has been the biggest gift to our sister and family. After a near death accident which left our sister with a permanent brain injury, we searched for the perfect facility that would meet her physical, mental and social needs. Our family is beyond grateful for the exquisite care our sister is receiving from the nursing staff, the activities coordinators and especially Lynda Connelly who has been a pillar of support and professionalism alongside the rest of the Executive & Business Staff. As Powers of Attorney for our sister, we are thankful for the loving, mindful, efficient and consistent care our sister receives and love the amount of time Portola Gardens puts into the wellbeing and socializing of their Residents with quality activities and homemade healthy food! If you need to locate your loved one into a facility, Portola Gardens and its beautiful grounds is a beautiful place and welcomes the family to be a part of their care & activities! They are the best! Thank You! 🙏

My family member resided at Portola Gardens for 4 years. The facility was renovated in 2019; it offers a pleasant and welcoming front room where residents are able to lounge and watch television. There is a sun room that offers plenty of natural light, where the residents can sit and relax; it also faces the backyard, where the residents can sit on one of the benches and enjoy the fresh air. The bedroom faced the backyard, which allowed privacy, a view of the trees and the ability to admire the flowers during the Spring and Summer seasons, and fresh air from the window. The staff is dedicated to their residents, some went above and beyond with compassion, kindness and personal/genuine care, which I will always appreciate and hold dear to my heart. Communication with the staff was beneficial in determining individual needs for proper care and practices. The staff was always available to talk and answer questions. The facility worked very closely with SF Health Department during Covid-19 to ensure the safety and well being for the residents and staff. Our family greatly appreciated their care for our loved one.
