My mother like this place. Everyone works like a one team. She feels well cared for and loved. It is not easy to please aging clients and this center is doing a great job!

I was in Poplar Hill rehab recovering from a fractured hip from March 28 through April 18, 2026. When I was admitted, Mr. Kamera Osmon, weekend supervisor, was especially helpful in my stay starting that weekend and following weekends. I would like to express my appreciation of the care I received there. The nurses, aides, physical therapists, occupational therapist, and all of the staff were very important in my ongoing recovery. Poplar Hill is a wonderful rehab / skilled nursing facility. My 91 year old father has been there a month and we couldn’t be happier with the care he is receiving. The staff are kind, compassionate and knowledgeable. The meals have been healthy, flavorful and full of variety. The facility is always clean. The residents seem happy and have a variety of fun activities to participate in if they so choose. I highly recommend this facility for someone needing short or long term care.

I have a loved one currently in this facility and the concern for proper care is a big concern here. She is a dyalisis patient who goes Mon,wed,Friday and she is scheduled for showers twice a week Tuesday, Friday before 9:30 a.m. due to dyalisis this day. She is lucky to get her Tuesday shower much less ever getting it on Friday morning. The real concern is that she has several wounds that need cleaned and dressed daily. I dated her wound dressing and when I came back to visit 4 days later the exact same dressing that I put date on was still on her. I have reached out to the director to voice my concerns and waiting to see what excuse I get. If you can keep your loved ones home I would definitely reconsider placing them in the care of this facility. Her room is so filthy the bed linen has not been changed since I did it , her room is full of ants. The communication is totally out the window you mind as well be taking to the wall. There has been a couple times that she didn’t receive her meds not to mention one time I was there visiting and the nurse gave her insulin and then came in 10 minutes later to give her more insulin. This place is just horrible I don’t know how the state has not shut this place down or at least get new management they need people who actually care about the patient instead of over charging the insurance companies

Poplar Hill is wonderful. I have sent 2 very dear friend’s parents here after having a positive experience with my own mother. The nursing staff was extremely caring. If I ever brought up concerns, they resolved them timely. The place looks clean, smells clean, and the staff is always smiling. Therapy was of course my mother’s favorite part of the day. They make working hard fun and are focused on the safety of the patient. I highly recommend Poplar Hill Health and Rehabilitation.
