This was my 1st visit to Your Facility. I came to visit a Friend who was there. I must say I was very impressed. Your Facility was very clean, the atmosphere was so peaceful. The Receptionist was very Friendly. The Room was very clean. If I had to be placed in a Facility it would definitely be Yours. Keep up the good work.

Plaza is the best overall short-term Rehabilitation facility in town. Clean, updated facility with staff who really care. Their admin staff was extremely helpful with the admission process, and all of their nurses and CNAs have been nothing but helpful and caring. Their PT and OT teams do a great job building strength and confidence as well. Highly recommend

I visited your care team for my sister everything went great. Everyone dates an awesome job. I recommended another patient to the Plaza and she loves it. My sister and I will come back and we want to bring my mom because it clean and everyone cares about the patient.

My grandmother was in the E wing (dementia unit) and I was very disappointed at her attention and care she received! I truly believe this nursing home was the cause of her death how can you let a dementia patient go days without water and food and send her out last minute and tell the family days later once it’s so worse you have to send her out! That manager has no compassion on that wing the nurse manager Jasmine doesn’t listen and take concern if the head person doesn’t care the workers won’t either! Please you all check on your loved ones make sure they are eating and have plenty of water because the attention and care is not at this place!

My dad was a resident at The Plaza Health and Rehab for three months, and I truly can't say enough wonderful things about this facility and its staff!!! From day one, Charolyn, the administrator, treated my dad like family. I live in Tampa, so I wasn’t able to visit every day or attend my dad’s doctor appointments. Charolyn went above and beyond to make sure my dad was never alone. She had staff accompany him to his doctor visits and always made sure I was called in so I could ask questions and stay fully informed. She answered every question I had and honored every request to ensure my dad was comfortable, safe and happy. My dad underwent a total hip replacement and was initially sent by the hospital to another rehab facility in Gainesville. Unfortunately, that experience was terrible, he hated it and begged to come home. I knew he wasn’t strong enough to be discharged, so I searched for another local facility and am so thankful I found The Plaza. Dr. Khuddus and his nurse practitioner, Brittany, were exceptional. They stayed on top of my dad’s complex medical needs and ensured his medications were correct for his stage 5 kidney disease, congestive heart failure, and diabetes. Dr. Khuddus personally called me multiple times to provide updates and always explained everything clearly, without overwhelming medical jargon. That meant the world to me! The entire staff was amazing and consistently treated my dad with dignity and respect. The therapy team was incredible!Isabella (PT) and Hara (OT) were patient, encouraging and thorough with each treatment session. They took the time to explain each task to my dad and provided me with weekly updates on his progress. My dad has been in several skilled nursing facilities over the years due to his health, and this is by far the best one he has ever been in. I truly wish we had known about The Plaza years ago when his health first began to decline. I highly recommend The Plaza Health and Rehab. Charolyn and her team are outstanding! It’s clear that excellence starts with her leadership and flows through the entire staff. I can't thank them enough for the exceptional care they gave my dad.
