Plaza Gardens

Plaza Gardens is a memory care and enhanced assisted living community in Anoka, Minnesota, part of The Plaza by Vivie campus. It is connected by an indoor skyway to Plaza CityView and serves residents who need additional daily support or specialized memory care in a residential setting.

Plaza Gardens offers 24-hour caregiver support, licensed nursing care available around the clock, and personalized care plans developed after nurse assessments. Residents receive three daily meals, personal emergency response pendants, and access to spiritual care and one-on-one engagement opportunities. The memory care neighborhood within Plaza Gardens is designed for safety, engagement, and dignity, with trained team members providing specialized programming including Music & Memory and a partnership with the Parkinson's Foundation.

The community provides private studio, one- or two-bedroom apartments with kitchenettes and private bathrooms. Residents enjoy shared common spaces for dining, relaxation, and conversation, as well as access to all amenities across The Plaza campus, including chef-prepared dining at The Pioneer Grill, a state-of-the-art wellness center, classes, and programs ranging from art and wellness to community outings and seasonal activities.

Located in historic downtown Anoka, The Plaza offers a walkable neighborhood setting where residents can experience the small-town charm of the area. Residents have the ability to transition between care levels within the community as their needs change, with most support needs accommodated through structured care plans.
