Title: Families Beware: My Uncle’s TBI Care Was Mishandled for Over Two Years My family is sharing this to warn anyone considering placing their loved one at this facility. What happened to my uncle—a veteran with a traumatic brain injury (TBI)—is something no family should ever experience. 1. Taken Across State Lines Without Proper Coverage This facility accepted my uncle from New York into New Jersey despite not being TBI‑qualified and without ensuring he had proper out‑of‑state medical coverage. They later claimed he “had no coverage,” yet they kept him anyway. No family was informed of this risk beforehand. 2. They Took Him Back After Saying They Wouldn’t When the family appealed for him to be transferred out, the facility stated they were not taking him back. But they did—and then used his “lack of coverage” as an excuse for the poor level of care he received afterward. 3. Over Two Years With No Documented Plan of Care For more than two years, the family received no clear documentation about: His actual treatment plan His physical therapy schedule What therapies were medically necessary for his TBI Any progress notes or rehabilitation goals A TBI patient cannot improve without structured, consistent therapy. He received none of that. 4. Left in the Same Room, Same Position, Same Wall for Years My uncle spent years in the same room, in the same position, staring at the same wall. No stimulation. No mobility plan. No rehabilitation. Then the facility had the nerve to tell us he “won’t be able to walk.” How could he, when he was never given the chance? 5. He Fell Twice Before They Took Action It took two falls—two preventable injuries—before the facility finally reacted. A TBI patient should never be placed in a situation where falls are even possible without proper supervision. 6. A Veteran Deserved Better My uncle served this country. He deserved dignity, proper medical care, and transparency. Instead, he was warehoused, ignored, and left to deteriorate. 7. Families Deserve the Truth If you are considering this facility, please understand that our experience shows a pattern of: Poor communication Lack of transparency Inadequate TBI care Failure to provide documentation Failure to follow through on their own statements Neglectful supervision No family should have to fight this hard just to get basic information about their loved one’s care. We have documentation, timelines, and proof of everything stated here. If the facility wants to challenge this, we welcome it—because the truth is on record.

I'm vacationing from Florida and Have a Family Member Who was Admitted today. I was pleasantly surprised on the treatment received by Staff Members. Sofia, from the entrance reception area instantly greeted me with a warm smile and attentiveness. Providing solutions to some of my immediate needs. I had a short visit with my Aunt and brief interactions with staff members located at the nurses' station and also one of the residents. As a Healthcare Worker Myself, Your establishment Executed All Green Flags! ❤️

I had the opportunity to tour the geriatric unit at Phoenix Center for Rehabilitation and Pediatrics as a representative of Visiting Angels, and had a very positive experience. Albina Tela, the Admissions Coordinator, was a gracious tour guide who took the time to show me the different floors and common areas while explaining the services they provide. She also introduced me to several key staff members along the way. The rooms, hallways, and dining areas were clean and well attended, and the residents appeared comfortable and content. Thank you to Albina and the team for the warm welcome.
