My grandma has been living here for more than a year, and the care has been AMAZING. She is treated as a whole person— not just as an old person with dementia. The staff is loving and very attentive. Doctor visits are regular, and all concerns are communicated quickly and clearly with me (POA and health care agent). HUGHLY recommended!!

Yes. Perham Living is a Medicare-certified home health agency, so Medicare Part A or Part B pays in full for skilled nursing, therapy and home health aide visits when a doctor orders them and the patient is homebound. There is no copay for covered home health visits. Medicare rates the agency's quality of patient care 3.0 out of 5 stars.
For Medicare to pay, yes: a doctor, nurse practitioner or physician assistant must certify that care is needed and sign a plan of care. Families can call the agency first, and the agency will usually contact the doctor to arrange the order. Ongoing help with bathing, meals and companionship is a different service, non-medical home care, which needs no referral.
Perham Living had 3 government inspection visits in the last three years, with 20 citations on file according to CMS Care Compare data. The full history, including what each citation was for and whether it was corrected, is in the inspection section on this page.
