The best Home Health organization I've experienced. Nurse Morgan Lewis, Physical Therapist, Leanna Kwok, Occupational Therapist, Debbie McCulley, were caring and professional. I highly suggest having this organization care for you or your loved ones. I even had a chance to speak with the Director, Jennifer Hert, and she proves it all starts at the top.

I can’t say enough good things about Countryside Home Health. They have been helping my mother get better since April of this year. The PT care that she received from Jake and Team was amazing. And Nancy Mount is probably the best nurse and one of the kindest people I know. We consider Nancy to be part of our family. Highly recommend!

Countryside has been providing home health care (nursing & PT) for my wife since the early part of this year. Kellie has been phenomenal attending to a wound that has taken several weeks to heal. Jake has followed up with a series of PT visits. He has gotten my wife to become more agile and actually had her standing on her feet, something she has not done since last year. And I'd be remiss if I didn't mention the support that I received from the office staff. My wife needed a flu shot. Lots of luck trying to find someone to come to your home to administer it! The local hospital system, my insurance companies, the County and other home health agencies all turned me down. But Countryside was the only agency willing to go the extra mile and make it happen.

I have been very pleased with the care my mom has received from Countryside Home Health Care. the staff has been kind, friendly and knowledgeable. Kelly Price RN has been fantastic. I credit her hands-on well as educational approach responsible for the healing of my mom's level two bedsores. Our OT Angie Lilly is so supportive and has walked me through countless problems, and Greg Luntz , the PT, can charm my mom into accomplishing more than I would have thought possible. i am so grateful that God put these people in my path.!.

We have been very impressed by the attentive and personable service provided by Tanzi Tayyab, CEO and Director of Nursing for Countryside Home Health Care, and her team. She came to the rehab facility shortly after she was notified we'd be needing home health care follow up for my husband. She was professional yet warm and caring, and we felt very much at ease after our initial meeting. My husband was originally to be discharged on a Saturday, but had made good progress so was able to leave the rehab facility on Thursday. That was no problem for Countryside Home Health Care--they quickly arranged for an evaluation by a visiting home health nurse and that was followed by appointments with both a physical and an occupational therapist. Each of these individuals did thorough evaluations and really focused on my husband's needs. They were knowledgeable as well as compassionate--things you really want to see in home health care. We cannot say enough good things about this group and highly recommend them in this community. Should we ever need home health care in the future, we will not hesitate to contact Countryside!
