THEY DONT HAVE THE STAFF THEY NEED , STAFF (CNA) ARE VERY UNPROFESSIONAL SOME ARE GREAT ) NO linen for The residence like washcloth towels, sheets, pillowcases, etc. a young lady used to work there bought her own stuff just to make sure they had it The receptionist make your day with a bubble personality her smile lights up the building Therapy is great, but I would not put my loved one in that building

If you value safe and consistent care for your loved one, avoid Old Southwest Rehabilitation and Nursing. Official inspection reports show repeated deficiencies, including failure to provide needed medications and timely laboratory services, and a lack of quality assurance in management practices. Residents have gone without enough food and fluids to maintain health, and the facility has been cited for failing to honor patient rights and to maintain a clean, safe environment. Management has consistently ignored both resident needs and corrective actions, resulting in unsafe and often neglectful conditions. Multiple recent complaints and fines reinforce the ongoing problems at this facility, making it a risky and unreliable choice for nursing care.

This community has definitely had its challenges in the past; however, it is a blessing that the new leadership and ownership is not only talented, compassionate, skilled and courageous, but most importantly, made it their mission to deliver the highest level of care and dignity. The current leadership team of this community has made astonishing improvements and puts the residents at their forefront. I highly recommend this community to anyone, ranging from short term therapy to long term care.

I have reviewed this facility a while back when it had a 1 star rating, then noticed all the uptick in 5 star reviews.. all given by team members or people asked to give positive reviews.. so keep this in mind if you are sending a loved one to this facility. This place is filthy (food, feces) on the floors, kept a little cleaner during the week when they have more staff. Infestations of bed bugs. Clothes, personal belongings anything your loved one owns will be stolen. The food is similar to overcooked prison food with many night of pb and js. Care is a little better during the weekdays but the night and weekend workers ( agency) are extremely rude and do not care for the residents often cussing them out or ignoring call bells. Showers are at the most performed once a week if they have the staff that wants to do it. If your loved one needs help with feeding, adls ect, good luck getting anyone to help on the weekend. The therapy teams are great but there is very little or no communication from the nursing team. If your loved one is sick you won't find out from them and if there is outbreaks of illness they do not let you know until you've exposed yourself unknowingly. The doctors of the facility are impossible to get a hold of as they use an agency and seem to never have any communication information for them. I have seen animals treated better than some of the residents of this facility. There is some good caring staff here but they are sadly outnumbered by rude uncooperative staff that get angry if you ask them for anything or completely ignore you. As this is a Medicaid facility most people have no choice but to be here and quickly decline from the lack of care. If you have a choice where to go, do not go here. 5/27/25 You got to love the generic replies to genuine problems with this facility. Of course I've contacted the facility with all the issues and they give you platitudes and false promises and continue to let your loved one remain in terrible care and conditions. If they can't feed or take care of themselves, they're not going to get the help they need or they're mocked by staff for their disability.

I've lived here for almost 6 years. This facility is wonderful. Staff is attentive, caring, loving. Activities are amazing & fun! Food is great! Cnas quick to answer call lights, take great care of you and listen to you. I have meet some amazing residents and they are like my family. We have lots of fun daily. Highly recommend!
