My aunt stayed here. The care team no matter how many times we said she wasn't going to do something they would try anyways. My aunt died in this facility but 2 days later called my 81 year old uncle and said we see she was discharged do you need home health. This was very unprofessional and very careless on this facilities part. My uncle just lost his wife and he gets this call. I dont recommend this place to anyone. If you love you loved one find somewhere better.

Yes, I live in Oak Park nursing home. I’ve lived here for a year and a half and they’ve changed administrators. The new administrator‘s name is Richard Gonzalez. He’s also the abuse coordinator well. Medication‘s were stolen from me Morphin and OxyContin. It’s been over a month now they’ve given me no resolution nothing has to what happens so the only thing I can think of is or if you get any ideas, call Dea and have them come check it out or the corporate office which they don’t give a number for so that you can’t report anything and they treat every person terrible I even asked him to try nicely kindness and it didn’t work. Mr. Richard Gonzalez is incapable of that to my knowledge. Thank you very much and I hope to hear some responses

Rolanda Shelton the Director of Nursing and Mary her ADON are so helpful and welcoming!! They made us feel comfortable with the team’s knowledge and ability to provide exceptional care.

My mom is a resident at this Center. She has been there for a little over a year. She has lived her BEST life here. The Activity Director recently had a Hawaiian theme party! It exceeded my expectations. The Staff treat the Residents with love and respect. Every party I gave attended i come home bragging about my Mom's nursing staff and aids. It comforts my heart to know that my mom is being taken care of around the clock.
