This place is amazing. My husband was in need of physical therapy and I chose Oak Creek based on location. I am so glad I did. The staff is warm and caring, they genuinely wanted the best for him. They treated both of us like we were part of their personal family. I would love to name each person individually that made an impact but I wouldn't want to skip anyone. Each person we came in contact with did everything they could to make a somewhat stressful situation so much easier. If you or any of your family or friends are ever in need of these services I highly recommend Oak Creek Rehabilitation I know you will not be disappointed.

Oak Creek has a very caring staff with new management in place who respond to resident concerns. It is a nice place for anyone who needs help with managing medications or needs someone to help take care of them.

(Editing) This review is not about the staff! I want to be very fail and accurate. The facility is very clean and looks very nice. The staff seemed very helpful and caring. This I was impressed about. If it were not for the noise, my first impression is a five star facility. I’m visiting a resident here. Unbelievably noisy! I feel like I’m in a cross between a Casino and a train station. Loud announcements, bings and alarms going off every few seconds, super loud televisions. I’ve been to many facilities and this is by a huge margin the loudest place I’ve ever seen. (Edited based on the response from the owner) If you can get the super loud paging system and series of alerts under control, it would make a huge difference. I get it. You have a system for managing the call button system that escalates when the staff does not or cannot respond right away. This is fantastic except the alerts are so loud and constantly going off. I actually work in this industry helping smaller hospitals to become five star facilities. One of the core tenants of our program is to create a quiet restful facility. I hope you work on this because you have everything else going for you. The staff seems good and it’s clean.

Absolutely terrible facility. We had my 100 year old grandma in here. When we came to visit we noticed basic human need such as teeth brushing and showers were neglected. The main case worker treated us like hell. When we visited and asked questions she decided to stay and supervise our visit. Donna then decided to cut our visit short when we started asking questions. We got my grandma lined up with another facility when they called for intake info Donna informed them that we (the family) had unrealistic expectations and were extremely difficult to deal with. We finally got the transfer lined up and unfortunately my grandma was having difficulty breathing and passed. Please do NOT let your love ones end up here!
