My grandfather was admitted to this facility for rehab following a serious fall resulting in hip and femur fractures. We were specifically told he would be placed near the nursing station and monitored closely, with a bed alarm in place due to his high fall risk. Despite this, he fell just 2.5 days after admission and was readmitted to the hospital. When we raised concerns, we were told that bed alarms are not routinely used because they may disturb other patients. This is extremely concerning given that fall prevention was the primary reason for his admission to rehab. Following his second hospitalization, we made the decision to transfer him to another facility. Additionally, during his hospital stay after the fall, there was no outreach from the facility to check on his condition. However, we did receive a call asking whether we would like to pay $800 to hold his room. Overall, our experience did not align with the level of safety and communication we were led to expect for a high-risk patient recovering from major fractures. I do not recommend placing your loved ones in this facility.

I spoke with the receptionist this evening I believe she said her name was Laura, she had the most helpful cheerful reassuring attitude. I truly appreciated the help she offered me especially with my mom being new to long term care. Much respect to all who had been assisting me in the journey.

My grandmother has been staying at this facility and I wish I had one nice thing to say about this place but I don’t. They haven’t updated me or my mother on any of her medical operations or medications, they have lost multiple of her belongings as well. Currently looking for other places other than this.

My late wife was treated like a dog. Food left in the roof of her mouth, I found her walking around unattended, like a zombie and several times I found her in bed, halfway off it, with her pajama top almost off and her naked body exposed. Terrible place.

My mom was there and the CNAs at this facility is over work, each time I visit my mom they only have 1 CNA working on the unit, in my one hour visit the Lpn nurse is gone off the unit most of the time, because she is working 2 until at a time this madness
