All the members of the rehab staff were kind, caring, professional. My team (Julia, Dorothy, Cath) was truly wonderful. Julia was the star! I was happy to see her each morning. She was cheerful, encouraging and motivating. I could not have made the progress that I did without the support and encouragement of this knowledgeable team. I will be forever grateful for help getting through the most difficult experience of my life. Now I'm home and continuing to improve thanks to the support and advice I received. As for direct care, Brenda was the best! She was patient, kind and caring.

This facility is not even worth one star. My mom went in to get stronger after surgery. She same out with various bedsores that were not there prior. They discharged her with a whooping UTI and blood at transfusion level. Fever sweating and torn skin bruising. It took an act of congress to change her catheter bag when I noticed there was a crimp in it. I repeatedly asked them to check her urine and that she seemed. Infused. I was going to check her OUT AMA but they talked me into staying an extra day and then she went to rehab ( with bedsores I hadn’t seen on her butte. ) and they said we would be responsible for her pt session if we checked her out. So I went and picked her up super sick and unable to stand. Called EMS and here we are . In the ER. I should have listened to my gut and taken her right out. Do not send your loved one here. Please.

They say if you want to know how clean a restaurant is go check the bathroom. On Saturday, December 27, 2025. I called this facility from the trauma center in Orlando, where my father is being treated for C2 fracture. The receptionist was gracious and transferred me to the nurses station because the administration‘s office was closed. I have one simple question which is, is this facility equipped to treat someone with a C2 fracture after surgery? The nurse answered the phone and I asked her her name to which she gave me reluctantly. I asked her that one question and she told me she could not answer me. I politely asked her why she couldn’t answer that one question where she is a nurse who is actually treating the patient there. Again, she refused to answer and told me I needed to talk to the admissions on Monday. I told her I was trying to be proactive to find a place for my father before Monday and I would appreciate her assistance. She told me to hold another nurse named Kim came to the phone. Kim was abrupt and told me that they do not answer any questions. I explained to her I wasn’t asking anything specific. I just wanted to know if this facility cared for people with neck fractures. Again she refused to answer that question and hung up on me abruptly. I guess there’s no reason to go inspect the bathroom in this facility. The nurses who took my call, had no empathy, no understanding for the situation, and I could tell they treated their patients the same exact way.

My mother Gaybe Delores Rogers was recently admitted for rehabilitation for about two weeks. Her experience there was among the best! She and my family would like to thank each and everyone of the staff for their compassion, professionalism, and kindness. Dorthy, Julia were absolutely the best! Mary and Brenda were the absolute best and all the nurses showed nothing but compassion and care! It really made a difference in her healing process! My mother and our family will never forget you all and the compassion you have showed through out this whole process! It really touched her heart as well as our family! We want to thank you all from the bottom of our hearts and wish each and everyone of you nothing but the best in life!

My 88 year old mom had a stroke in 2021. We brought her to Moultrie Creek Nursing and Rehab, thinking she might have 3 or 4 months to live. Now, 4 years later, under the high quality care, therapies, attention, and love of all the staff here, she is still with us! Even though she's an invalid, the staff treat her with respect, love and kindness. They use the hoyer lift to get her up everyday so she can enjoy her activities: Bingo, music groups, church, meals in the dining room and day room, visits outside in the courtyard (which is an outdoor refreshing little oasis). With mom's severe allergy to gluten, the kitchen staff are gratefully willing to be very careful and creative about mom's meals! The laundry and housekeeping teams do an excellent job of keeping mom's clothes clean and available in her closet, and of keeping the facility clean! The first things you'll notice when you enter the building, after greetings by the cheerful and friendly receptionist, is the total lack of 'nursing home smells'. 5 stars is for the overall high quality of care given to people with immense needs. Thank you for doing the jobs for our loved ones that we family members are not equipped to do. Thank you.
