Mission De La Casa

Mission de la Casa is a 24-hour skilled nursing facility located in San Jose, California, in the heart of Silicon Valley near the eastern hills and close to highways 101, 280, and 87. The independently and privately owned community provides short-term post-acute rehabilitation, long-term custodial care, and memory care services.

The facility offers comprehensive nursing and rehabilitation services including 24-hour skilled nursing, post-acute rehabilitation with physical therapy, occupational therapy, and speech-language pathology services seven days a week, and wound care. Staff are available to guide residents and families in accessing financial resources, and the community accepts various insurance providers.
