This is the best SNF my dad has been at. His nurses reach out to my mom and I every time he gets sent out to the hospital. The staff has care meetings with us and informs us about everything that goes on. Everyone we talk to is very nice. They are there whenever we need something. Every other facility hasn’t met our expectations like this one. Highly recommend.

This facility is great ! I admire all of the staff, especially the male nurse Alexis. He has great communication skills and really puts his whole heart into taking care of these patients.

Unfortunately I can't give it a less than zero score but if I could I would my mom went in there on Friday she's got a severe bed sore on her back end that they neglected her all weekend never cleaned her up never turned her over now she's in the ICU at a hospital with an infection in her bone due to that bed sore now mind you she did not get the bed sore at this facility but they knew it was there and they neglected to take care of her all weekend long and the smell in that building is atrocious from the time you walk in the door and then the smell is even worse when you get on the elevator it's so sad that we trust people to take care of our loved ones and they can't even do their job cure incompetence or laziness I don't know what it is but it's pathetic and they should be ashamed of theirselves if you don't want to do your job you shouldn't be in this line of work also they never got a suction tube for her so when she ended up at the emergency room they had to replace her trait too because it was so clogged up and she went into this facility on a Friday from the hospital in a hospital gown and when she ended up back at the hospital on Monday she was in the same hospital gown she came there in and then the other woman in the room is got epilepsy they had her in a bed with no railings and she actually fell out of the bed they came in and couldn't find her and they found her on the floor on the other side of the bed tell me how incompetent is that

Mariah the front desk agent needs to be fired or retrained. I called to ask if they offer speech therapy, she left me on hold for 10 minutes because she said she didn’t know. I called from a different phone number asked her the same question she said no and I asked if she’s the same person who had me on hold and she said yes. I asked why didn’t she come to the phone and tell me and her response was “I got busy”… clearly not that busy if you can pick up the other phone to answer. This is so unprofessional and she also has a nasty nonchalant attitude. Not the type of person who should be working at this type of place, let alone be answering the phones. Especially if she doesn’t even know what services you offer there like??? That was frustrating

My mother has been at this facility for 3 months. I can’t say enough about the staff on the second floor; the respiratory team, nurses and CNAs, the rehab team and activities director are amazing, very caring and compassionate providers. The front desk staff, housekeeping and the administrative staff is commendable as well. Having to place your loved one in a long term care facility can be a difficult, life changing and emotional experience. Having a facility such as Meridian Monte vista is a gem. Special mention: ADON Tyrone for being communicative and responsive, RT Carmen, & Rehab director Ray who go over and beyond to help mom and Activities director Catina who visits with mom every day and brings her coffee. May God bless and keep you all.
