Meadow Crest, as it’s lovingly referred to as, is a full-fledged NURSING HOME & REHAB located in the BETHAL Park/South Park area. It’s nestled in among other living spaces on a rural type of road…drivers tend to drive slowly and carefully…as they look for their destination. The workers here strive to make the living spaces as clean as possible. It’s an older facility and to me, looks like it could have been a “motel” sometime in the past! The grounds are well kept and maintained. They’ve a small sitting area with comfy outdoor tables and chairs and even umbrellas! It is a secure building as well; a code is necessary to get in AND OUT of the building! You can visit your friend or loved one any day of the week and all staff is friendly and kind. As I said, this is an older nursing home/rehab, but it’s “homey” style (vs institutional) helps makes it a decent choice when needed.

- excellent care my brother is getting at meadowcrest - staff are so kind to him and the other residents - food, they serve is so good and a healthy balance of food on the trays - dietary staff not only provide the meals but will accommodate patients needs, they are cheerful and respectful to likes and dislikes for the menu - activities director is attentive to the needs of the residents to provide a balance of activities for them - nurses and aides treat the residents with care and respect. they are generous with answering questions by family members. they are kind and patient. -when i observe the staff interacting with him its so good to see him encouraging him and engaging with him in conversation and how it makes him smile, makes my day - therapy staff provide excellent services, they work with each resident to individualize the therapy and explain the "why' behind the activities - housekeeping and maintenance work is so important, cleaning washing clothes, maintaining issues, and they always interact with the residents - if you have questions the administration staff and social worker are on site and willing to answer as needed (like an open door policy)

I have spent a lot of time in Meadowcrest Rehab Center. The care I received was very professional. The nurses were always there to take care of my Illiostomy Bag when needed. I especially want to thank Lynn and Patty for taking the time and making sure I had the right equipment to take care of my Bag. I would also like to thank the following Nurses for their continued help and support, I want to thank all the Nurses and CNAs and Staff for all the help they gave to me during my stay. I want to give a special thank you to the following people.the Nurses Brea, Amanda, Monica and Linda. I would also like to thank a couple of the CNAs who constantly checked on me and took care of my Bag and needs Brandon and Elliott worked the afternoon and evening shift and they would check my bag constantly and empty it without a problem so that the bag wouldn’t leak or break on me. The Therapy I received was great. I came in not being able to walk and they worked with me and now I am able to walk again, a little slower but I am able to get around. They have a wonderful Activity Center run by Kristy who is the director of Activities and she is exceptional. We played Bingo, did crafts, and even got our nails done. There was something to do every day. I have an up coming surgery in a couple weeks and if I have to return to Rehab for a few weeks I will be returning to Meadowcrest for therapy. Thank you to everyone for your help and support during my stay.

I came here in August 2025. I had my hip replaced, and since then I have been here three more times. The staff here are friendly and considerate—from the aides, nurses, doctor, PT and OT therapists, office workers, housekeeping, dietary, and activities staff. The aides were friendly and considerate. They changed bed linens daily, supplied what was needed to wash up in the morning, delivered dinner trays, and helped get you anything you needed. The nurses were friendly, considerate, and knowledgeable. They passed out medications and answered any questions, or would find out for you if needed. The doctor is great and very knowledgeable. He understood what was wrong, listened to any questions you had, and had very good bedside manner. The PT and OT therapists were friendly, considerate, and knowledgeable in their field. They provided the proper therapy required. Housekeeping was friendly and considerate, cleaned rooms daily, kept the facility clean, and did laundry when needed. Dietary staff are friendly and considerate. Hot food was hot, cold food was cold, portions were standard, food was made in house, and desserts were homemade. Office workers are friendly and considerate and would answer any questions and help you out with anything you asked. Activities staff were friendly and considerate, with something to do most days, from bingo to pizza day. They also made sure to celebrate our birthdays. The staff at Meadowcrest have pleasant attitudes and are friendly and considerate. I would tell anybody that Meadowcrest is a great rehabilitation facility to go to!

DO NOT TAKE YOUR LOVED ONES TO THIS FACILITY. The website is a LIE. All but one photo online is a stock photo. My father was there for 2.5 days and all of the progress he made at the hospital had been undone. His condition rapidly deteriorated. The male nurse on staff at the time of an emergency would not call EMS and we (the family) needed to force it and make it happen ourselves. Thankfully we were able to get him out. He is now in a new facility that can meet his needs. This place is unsanitary and unsafe.
