Stopped to visit a friend today. First time at the facility. First impression walking in the lobby everything is clean and beautifully decorated. Check-in was easy. Spoke to four folks that work there and they were all lovely and helpful. My friend has been there for at least a month said that the food is good. Everybody treats him wonderful and he could not be happier. That made me very happy that he said that!!

I love the attention the Admin give to the staff who in turn pas it on to the residents. They spare no cost in celebrating holidays and occasions I have never heard of. I am happy we chose this facility for mom in this season of her life.

Love going into Madison Health and Rehabilitation! Staff is always friendly and always willing to help with anything. Residents seems to be so happy there. Very clean building

First let me say that at this point, I would not recommend this facility to anyone or their friends or family members. I wanted to wait a few weeks after we left before I did this review so that it wouldn't be done out of emotionalism but it would be fair and well thought-out. My husband was there in October 2025 and there were several issues which will be listed below. 1. We were persuaded to come there under false pretenses by the admissions coordinator who did not give us all the information about our transfer. He did not tell us we could only be there 20 days and he also lied and said I could stay in the room with my husband (i could not). I had to get a hotel. 2. The first day of arrival we were told by a nurse that my husband was in the wrong facility if he needs attention because In the state of Florida the ratio is 1 nurse per 40 patients and 1 cna per 20 patients. A few days later the same nurse walked out of the room and said " im not doing this" when my husband questioned the medication he was being given. Tur s out, the medication wasn't right. 3. Several nights I had to be awakened to leave my hotel room to go and assist my husband in the middle of the night becsuse the staff wouldn't answer the call lights. One nurse told him " I heard your wife like to do everything for you so I would rather her do it" One night in particular it took such a toll on me that I ended up calling the social worker at home (Tori) She was extremely helpful. After arriving at the facility I still had a hard time getting in because the staff was walking around with ear buds in and couldn't hear me ringing the bell 4. One Sunday night a staff member had her child in the nurses station where he was standing in the chairs, destroying the desk and hittiing staff with a toy as they walked by. Some residents that were sitting around the station were getting hit at also. These issues were promptly brought to the appropriate people. And I must say that they were addressed with us. However, we weren't around long enough to see if anything changed. Madison Health and Rehab has great potential because they do have some great staff members there. However, at night, that place turns Into a jungle. I would like to thank Mrs Tori (social worker) for showing so much concern and respect for us, Courtney (nurse), we had a few issues in the beginning but she became an absolute pleasure to work with, also the director of nursing ( i can't remember her name), Hillary and Mr. Ron.

Friend was there for several weeks. I visited 7 - 10 days over a 5 week period. I live near Atlanta. It was a nice small place. Staff seemed very nice and competent. Room cleaning could be a little more often.
