I spent 6 years at Lake Park of Madison H&R, I went through the Covid pandemic here as a Resident , I have no complaints , The staff is true to the small town of Madison’s friendliness and Christian views . Thefood was Excellent and Activities was just as good . Administration knew everyone by name and always reached out with encouragement and Love , TheNurses were very helpful in describing any problems I had . All staff .worked as one led by fair and loving Leaders , Iwould definitely recommend anyone in need of LTC to choose LakePark Madison H& R Center And the local Churches that visit with Christmas Carols and treats make the whole life here fullof great memories Imoved onttobecloser to family , Ihad verylittletodo with Rehab but the few times I had some , TheLadies were Great Motivators

My Uncle has been here for 2 years now and seems to like being here since he isn't able to go home because of his health. Now my Aunt is here and they get to see each other every day, they are in their 80"s. The facility is clean , it seems like they just need more staff. One lady we saw in passing needed help getting dressed, her door was wide open and no one seemed to worry about her. The nurse we talked to just started that day. Hallway 600.This was on 10/27/2025. The nurse taking care of my Aunt did show her respect when helping to get her ready for lunch. I did appreciate that as I haven't seen my Aunt in this condition before where she can't take care of herself now.

I would like to thank all of the staff at Lake Park Nursing Home and Rehabilitation Center. My Mother in-law was a resident for three months and the care she was given was absolutely wonderful. The nurses are so caring and accommodating to everything that my mother-in-law asked or wished for. We can not thank all of you enough for being who each of you are.

When my mother fell ill and could not take care of herself anymore she was placed at Lake Park at first she did not want to be there because it was a little ways from home but she ended up loving every minute of her stay there. She was so happy there and well taken care of she had nothing but great things to say about Lake Park. If I remember correctly for her birthday they gave her a tiara birthday crown flowers and some other things and she was the birthday Queen. Just want to say thank you Lake Park for treating my mother with respect and dignity. I highly recommend Lake Park for anyone who has the difficult decision of placing their loved one in a rehab/home.

My visit with my mother was great. It appears that she's being taken care of in a caring manner. The reason for my rating is that upon our departure there was no one at the front desk for security purpose. There were two ladies that said they had been waiting for a while to be escorted to see there love one as my family was also signing in. The young lady returned having gone out to get something to eat. There should be some coordination so she can leave while someone sit in not leaving the entry unattended. The concern is that ANYONE could have entered the building with not so good of intentions placing all the patients in the facility in danger.
