If I can give them zero stars I would. This facility basically killed my father. I had a nurse who worked there tell me to get a lawyer, so that’s what I did. However, it’s hard to fight a corporation that owns this facility that made 2 billion dollars last year. DO NOT send anyone here or I place yourself at this institution

My grandmother stayed at Lake Port Square for rehab for a couple of weeks, and she absolutely loved it. The staff took such great care of her, she felt safe, supported, and truly at home. The environment is warm, welcoming, and beautifully maintained. She was so happy with her experience that she chose to move into assisted living here after her rehab stay. We’re incredibly grateful for the compassionate care she received and the peace of mind it’s brought to our family.

Lake port square is a very clean and beautiful place. We work together as a team to make sure the residents feel at home and are well taken care of. The doctors here are very attentive and quickly responsive to make sure the residents stay happy and healthy.

Had a very good experience at Lake port square health center. Everyone was very friendly and the rehab was great. Big shout out to Rose who was my CNA, Leigh in activities and Moe and Dee in the therapy department. And the food wasn’t bad either.
