My grandfather recently stayed at Lake Pleasant for his rehab, and our family was very pleased with his time there. The recent renovations look great, and the facility feels clean and welcoming. Most importantly, the staff was caring, attentive, and supportive throughout his recovery. We’re grateful for the excellent care he received and would highly recommend Lake Pleasant Post Acute.

Our family had a very concerning experience with a loved one’s stay at Lake Pleasant Post Acute Rehabilitation (Jan–Feb 2026). Three issues stood out: 1. Condition on arrival during visits: On multiple visits, we arrived to find our loved one not positioned comfortably or safely and without basic items consistently within reach (call light, phone, and water). We often had to ask staff for help immediately. 2. Communication with family/MPOA: Changes in condition, care needs, and the care plan were not communicated consistently to the family or the legal decision-maker (MPOA). We repeatedly had to initiate follow-ups to understand what was happening. 3. Records and flow sheets (repositioning/hydration): When we requested documentation such as repositioning and hydration flow sheets/logs, we did not receive complete records in a timely way. That made it difficult to verify care and ensure continuity. I strongly recommend families visit frequently, request a written care plan, and ask for written documentation (including repositioning/hydration logs) early in the stay.

My father in law had an incredible time here. It was the first time a place actually felt clean, well kept, and the staff exceeded our needs. The place was spotless and the newly renovated rooms made it super nice. Couldn’t have asked for a better experience, and would recommend this to all of my loved ones

Lake pleasant post acute rehabilitation center is a great place for rehabilitation. I enjoy being here and all my nurses are very compassionate and helpful. The administration is great and make it easy to want to be here. Thank you Saul, Jen, Jessica,Diana,Briana,Mariah, Cici,Erin(nurse),Kennedy(nurse),Amara(nurse),Teryn(nurse), Veronica(nurse),Denise(nurse),Regina(nurse),Patti (nurse),Pamela (nurse),Dulce(night nurse),Mychelle(night nurse),Angela (night nurse),Crystal(night nurse),Becky, and Heather. Jimmy in house keeping,and also the CNA’s, Sara ,Reilly, and Tessie, Baliegh,Adelia,Jess,Tereza,Mia,Vaelyn,and Temaya. Last but not least the ladies in activities , Julie , and Desiray ,I love them all. Way too many to list. The whole squad is awesome.
