If I could give 0 stars I would. The state needs to shut this place down. There's no communication between the staff and families. They do whatever the hell they want to do. And when you try to discuss your frustration they try to gas light you and become facetious. I wouldn't even send my dog here.

I was having a hard time finding a place to place my father. I was a little skeptical because of Some of the previous reviews on here, but I must say this place is a diamond in the rough! I had my dad originally in a top-notch place. When he got out, he broke his back and needed to be replaced, and I couldn’t get him back in there. We ended up here. I could not be more impressed with the love this place is filled with all the way through from front desk, Maintenance, Dietetics, AIDS, nurses and doctors and all Administration…They treat every resident like their own family like a parent or a grandparent. He does not wait if he’s sitting on the potty. He gets meals. He absolutely loves. If he’s not well, they are on it in a minute! They are so good to all of the residents. They also love & Honor their veterans. They are under new ownership for the past few years and have recently redone their staffing and they have done an amazing job. Please give this place a chance. If you’re loved one needs rehab or Care in general, please consider them. I absolutely do not regret my decision to bring my father here. He literally told me it feels like home to him.. I know he’s safe and he LIVES the food! These things not only mean the world to me, but bring me great ease and peace of mind! They are like our family. Thank you Lake Mariam Health and rehabilitation staff for the best kind of care! This is exactly the way our loved one’s should be cared for! As a very anal about good care health care worker I know! ♥️

For the past several weeks, my father had been exhibiting signs and symptoms of illness. These concerns were communicated to nursing staff. Despite this, his condition appeared to progressively worsen without appropriate escalation. When my daughter returned on Monday, February 16, 2026, I found my father in bed, non-responsive, with labored breathing and gasping respirations. My daughter immediately requested that 911 be called. She was informed that staff were “waiting on doctors” and other directives. She reiterated that he needed to be sent out immediately and followed up directly with the Director of Nursing to confirm this request. Approximately 15–20 minutes later, EMS arrived and transported him in critical condition. At that time, he was mottled, had a temperature of 107.4°F, and oxygen saturation of 79%. He remained critical upon arrival at the emergency department, was transferred to the ICU, placed on a ventilator, and passed away at 2:50 p.m. on February 17, 2026. This is NEGLECT! I would not put your lived one here. While there are some great CNAs and Nurses, very few...not all are the same!

Avoid putting a loved one here. They will tell you that they are turning things around and making improvements. Basically they will tell you everything you want to hear when you’re under duress. My dad was there for a month. My sister and I were there everyday. We spent a minimum of 8 hours a day between the two of us. There is no communication between staff, constant staffing changes, lack of basic care, unsanitary conditions and the list goes on. Even after filing several grievances and meeting with the entire team, things were still a mess. There is one nurse in particular, Gail, who is the most vile and rudest person I have ever encountered in the medical community. My dad was self pay and we paid 6 weeks in advance. We were told if we wanted to move him, the refund would take about 10 days. He passed away before we were able to do so. It’s been 6 weeks and still no refund. Their accounting practices are questionable. After we paid 6 weeks in advance we received a bill two weeks later stating they forgot to tell us that there was a price increase in December and we owed more. Absolutely not! My attorney is looking into their background with finances. Stay away from here if possible. If we had so much trouble and we were there everyday, imagine what happens to your loved one when you can’t be there. There were a few good staff members but they could only do so much.

My mother has advanced Alzheimer's, and life is tricky. She recently stayed in the memory care facility at Lake Mariam Health for a 5-night respite stay. I was very impressed. Unlike some other facilities I am acquainted with, Lake Mariam was very well-staffed, and the staff was visible, engaged, friendly, and eager to help. My mother received excellent, compassionate care at Lake Mariam for which I am very thankful.
